Background: Hepatitis C virus (HCV) therapy has been evolved over years and many parameters were used to evaluate such therapy particularly genotyping. In North Africa and Eastern Mediterranean the genotypes vary from most of world and rarely studies were conducted to assess the influence of such genotypes on the HCV therapy. Aims: This study was designed to determine; the role of HCV genotyping in assessing the efficacy of interferon therapy and to analyze the rates of sustained virological response (SVR) in INF and PegIFN-based regimens according on HCV genotype infecting Libyan patients with chronic hepatitis C infection. Methods: A total of 479 patients with chronic HCV registered at Tripoli Medical Centre were treated with 'INF alfa or PEG-INF-Pegylated INF alfa 2a in combination with ribavirin' for a five year period. These patients were registered and followed up from January 2007 to October 2012. The information were reviewed and data were collected from each patient regarding age, gender, ALT level, and viral load, viral genotype using qualitative PCR. The statistical analysis were carried using SPSS version 11.5. Results: Of the patients studied 86 patients were treated with INF based regimen, only 54% of them had end treatment response (ETR) and 28% had SVR. Off 143 patients treated with peg-INF alfa 2a based therapy; 69% had ETR and 36% had SVR. The SVR of Peg-INF based regimen was higher than INF based regimen in all genotypes except for genotype 4. The relationship between SVR and gender was significant in patients who were given INF based therapy comparable to PEG INF based regimen, though the relationship between SVR and age, basal viral load and basal ALT were also reported in both regimens. Conclusions: HCV genotyping has been found to play an important valuable role in determining the efficacy of Hepatitis C therapy. SVR vary according to the HCV genotype involved. HCV genotype 1 and 4 were found to be the prevalent resistant genotypes infecting Libyan patients. Such findings are particularly important in guiding the clinical therapy of patients infected with hepatitis C virus.

Background The burden of hepatitis C virus genotype 4 (HCV-4) is high in Africa and East Mediterranean countries. Previous reports estimate sustained virologic response (SVR) rates in HCV-4 to be ~20–70%. However, many of these studies are limited by different study designs and small sample sizes. Aim Our aim was to evaluate treatment outcome and host/viral factors on SVR in HCV-4 patients treated with pegylated interferon and ribavirin (PEG IFN+RBV) in a systematic and quantitative fashion. Methods A comprehensive literature search in MEDLINE and EMBASE for ‘genotype 4’ was conducted in November 2013. Abstracts from AASLD, APASL, DDW, and EASL in 2012/2013 were reviewed. Inclusion criteria were original studies with ≥25 treatment-naïve HCV-4 patients treated with PEG IFN+RBV. Exclusion criteria were co-infection with HIV, HBV, or other genotypes. Effect sizes were calculated using random-effects models. Heterogeneity was determined by Cochrane Q-test (p-value <0.05) and I-squared statistic (>50%). Results We included 51 studies (11,102 HCV-4 patients) in the primary analysis. Pooled SVR was 53% (95% CI: 50–55%) (Q-statistic = 269.20, p<0.05; I2 = 81.43). On sub-group analyses, SVR was significantly associated with lower viral load, OR 3.05 (CI 1.80–5.17, p<0.001); mild fibrosis, OR 3.17 (CI 2.19–4.59, p<0.001); and favorable IL28B polymorphisms, rs12979860 CC versus CT/TT, OR 4.70 (CI 2.87–7.69, p<0.001), and rs8099917 TT versus GT/GG, OR 5.21 (CI 2.31–11.73, p<0.001). Conclusions HCV-4 patients treated with PEG IFN+RBV may expect SVR rate of ~50%. Lower viral load, mild fibrosis, and favorable IL28B (rs12979860 CC and rs8099917 TT) are positively associated with SVR.

Notre travail a pour but d'étudier les aspects épidé-miologique, diagnostique et thérapeutique de l'hépatite C chronique. Patients et méthodes : Il s'agit d'une étude clinique prospective réalisée, d'octobre 2005 à décembre 2009, dans le service d'hépatogastroentérologie. Ont été inclus les patients anti-virus de l'hépatite C (VHC) et ARN-VHC positifs. La quantification de l'ARN-VHC a été réalisée par PCR en temps réel. Les malades ont été traités par la combinaison interféron pégylé alfa-2a et ribavirine.Résultats : Au total, 46 patients ont été inclus dont 30 hommes (65,2 %). L'âge moyen était de 47 ans. L'hépatite C a été découverte à l'occasion d'un bilan de santé (37 %), d'une cytolyse (34,8 %), d'un don de sang (15,2 %). Cinq patients (10,9 %) avaient des antécédents de transfusion sanguine. L'examen physique était normal chez 42 patients (91,3 %), quatre patients (8,7 %) avaient une cirrhose. La charge virale moyenne était de 960 978 UI/ml (5,9 log). Les géno-types 1, 2 et 5 ont été retrouvés avec une prédominance du type 2 (71,1 %). Dix-huit patients naïfs ont été traités. Tous génotypes confondus, la réponse virologique soutenue (RVS) a été de 81,2 % dont 92,3 % pour le génotype 2. Trois cas de rechute et deux arrêts de traitement à cause d'effets secondaires sévères ont été observés. La baisse du taux d'hémoglobine a été en moyenne de 3 ± 1,7 g/dl. Une neutropénie inférieure à 750/mm 3 a été observée chez six patients (33,3 %). Deux patients ont présenté une thrombopénie inférieure ou égale à 100 000/mm 3 (11,1 %). La dose d'interféron pégylé a été réduite chez huit patients (44,4 %), celle de la ribavirine chez quatre (22,2 %). Une dépression a été diagnostiquée chez trois patients (6,5 %).
