Post-stroke depression (PSD) is a common complication after stroke. PSD is associated with emotional disorders and psychological dependence, which are potential risk factors for stroke recurrence and suicidality. This study aimed to perform an umbrella review of therapies for PSD through a comprehensive literature search. A systematic search was conducted in the PubMed and Web of Science by two independent authors. We examined the Hamilton Depression Scale (HAMD), Activities of daily living (ADL), Neurologic function as efficacy endpoints, and the incidence of adverse events as safety profiles. Seventeen eligible studies, including 267 clinical trials were included in this study. The results showed that High-Frequency Repetitive Transcranial Magnetic Stimulation (HfrTMS), Acupuncture/EA+conventional treatment, Escitalopram, Modified Sini San, Moxibustion, Xiaoyao Formula, Paroxetine, Chinese herbal medicine, Exercise, Citalopram, and Cognitive behavioral therapy are beneficial for improving the depression symptoms of patients with PSD. HfrTMS and Sertraline may have an impact on slowing the scores of activities of daily living or neurologic function. In addition, Acupuncture/EA+conventional, Escitalopram, Citalopram, Sertraline, and Fluoxetine showed no serious adverse events in PSD patients. Our study demonstrated that 11 treatment methods can effectively improve the condition of PSD patients.

Background: Depression is a common complication after stroke and is closely related to the poor prognosis of stroke. Antidepressants are the priority drug in the treatment of post-stroke depression (PSD), but there are dependence and adverse reactions. Danzhi Xiaoyao Powder has a good effect on depression without obvious adverse reactions. At present, there is a lack of rigorous randomized controlled trials to evaluate the clinical efficacy of Danzhi Xiaoyao Powder in the treatment of PSD. Methods: This is a prospective, randomized, double-blind, parallel controlled trial to explore the efficacy and safety of Danzhi Xiaoyao Powder in the treatment of PSD. The participants were randomly divided into treatment group and control group. The treatment group used Danzhi Xiaoyao Powder combined with escitalopram oxalate, and the control group used Danzhi Xiaoyao Powder simulant combined with citalopram oxalate. The two groups were both treated for 8 weeks and followed up for 3 months. Observational index includes: Total response rate, Hamilton depression scale, Barthel index, national institutes of health stroke scale, the modified Edinburgh-Scandinavian stroke scale, Incidence of adverse reactions. Finally, SPASS 22.0 software was used for statistical analysis of the data. Discussion: This study will evaluate the clinical efficacy of Danzhi Xiaoyao Powder in the treatment of PSD. The results of this study will provide reliable evidence for the clinical use of Xiaoyao Powder in the treatment of PSD. Background: Depression is a common clinical psychiatric disorder that is responsible for health-related disease burdens globally. According to traditional Chinese medicine (TCM), mental disorders and qi stagnation are important pathogenic mechanisms of depression. The Chaihu Longgu Oyster Decoction, which has been documented in the Shanghanlun (Treatise on Typhoid), is widely used to treat various affective disorders. Methods: Network pharmacology and molecular docking technology were used to investigate the material basis and mechanism of action of the Chaihu Longgu oyster adjusted decoction in treating depression. The main pharmacological substance bases, possible targets, and pathways of Chaihu Longgu oyster adjusted decoction in treating depression were visualized by constructing a "component-pathway-target" network. Results: Quercetin, 7-methoxy-2-methylisoflavone, baicalein, kaempferol, and lignan are the main practical chemical components in Chaihu Longgu oyster adjusted decoction. The Chaihu Longgu oyster adjusted decoction regulates 74 protein targets and 142 pathways associated with depression. Its molecular mechanism involves inhibiting neuroinflammation and improving neurotransmitter function, neuroplasticity, etc. Conclusions: The underlying mechanism of the anti-depressive effect of the Chaihu Longgu oyster adjusted decoction may involve neuroinflammatory response reduction and improvement of neurotransmitter function and neuroplasticity. This study revealed the mechanism of action of the Chaihu Longgu oyster adjusted decoction in the treatment of depression through network pharmacology, which provides a scientific basis for clinical application.
