Efficacy and Safety of a Novel Cholesteryl Ester Transfer Protein Inhibitor, JTT-705, in Humans

Abstract: Background-Cholesteryl ester transfer protein (CETP) mediates the transfer of neutral lipids between lipoproteins. High plasma levels of CETP are correlated with low HDL cholesterol levels, a strong risk factor for coronary artery disease. In earlier studies, JTT-705, a novel CETP inhibitor, was shown to increase plasma HDL cholesterol and to inhibit the progression of atherosclerosis in cholesterol-fed rabbits. This study describes the first results using this CETP inhibitor in humans. Methods and Results-In … Show more

“…80 The highest dose induced a 37% decrease in CETP activity, a 34% increase in HDL, and a 7% decrease in LDL, with no change in triglyceride. An increase in HDL 2 , HDL 3 , and apoA-I paralleled the increase in total HDL.…”

“…Animals were fed JTT-705-containing mouse pellets for 6 days, and each animal received an estimated dose of 0.6 mg of compound/day. On a body weight basis, this dose is ϳ2 times higher than the dose shown to be effective for inhibiting CETP activity in humans (25). After the end of the feeding period, animals were intraperitoneally inoculated with LPS from P. aeruginosa PAO1, and serum levels of TNF␣ were determined 60 min after LPS inoculation.…”

“…32 This contrasts with the reduced CETP activity on pharmacological inhibition that is accompanied by an increase in CETP plasma concentration, probably because the interaction of the small molecules with the CETP protein disturbs CETP clearance from the circulation. 10,11 When these considerations are taken into account, care is warranted when extrapolating our finding that low CETP concentrations are associated with a moderately reduced risk for CAD in healthy individuals to the putative effects of CETP inhibition on CAD risk in patients. The ongoing trials assessing the effect of CETP inhibitors on surrogate markers of CAD and clinical end points will have to answer this question.…”
