Bacterial infections are frequent complications in patients with liver cirrhosis. Cirrhotic patients present abnormalities in both innate and adaptive immune responses, including a deficient neutrophil recruitment to infected sites. The purpose of this study was to assess neutrophil-endothelium interactions in cirrhotic patients and evaluate the effects of G-CSF on this process. We studied neutrophil adhesion and transendothelial migration in 14 cirrhotic patients and 14 healthy controls. We also analyzed neutrophil expression of the adhesion molecules CD62L and CD11b in whole blood by flow cytometry. Cirrhotic patients expressed higher levels of CD11b than healthy controls, whereas CD62L expression was significantly lower, suggesting exposure of neutrophils to activating agents within the bloodstream. Neutrophils from cirrhotic patients showed increased adhesion to both resting and tumor necrosis factor alpha-stimulated microvascular endothelial cells and decreased transendothelial migration. Granulocyte colony-stimulating factor (G-CSF) (100 ng/ml) significantly enhanced neutrophil adhesion to microvascular endothelial cells in healthy controls but not in cirrhotic patients. G-CSF also significantly improved neutrophil transmigration in cirrhotic patients and healthy controls. In conclusion, cirrhotic patients exhibit increased neutrophil adhesion to microvascular endothelium and deficient transendothelial migration. G-CSF enhances neutrophil transendothelial migration in cirrhotic patients despite having no effect on neutrophil adhesion. Therefore, G-CSF may be able to increase neutrophil recruitment into infected sites in these patients

Objective. To establish whether the clinical efficacy of pulse methylprednisolone (MP; 1,000 mg intravenously) is related to the modulation of proinflammatory cytokines within the peripheral blood, synovial membrane, or synovial fluid compartments.Methods. Eighteen patients with active rheumatoid arthritis (RA) were studied. Peripheral blood (11 patients) and knee synovial fluid (9 patients, 10 knees) were obtained before and at 4 and 24 hours after MP therapy. Interleukin-lp (IL-lp), IL-8, and tumor necrosis factor a (TNFa) were measured by enzyme-linked immunosorbent assay and biologic assays; prostaglandin E, (PGE,) was measured by competitive radioimmunoassay. In 10 patients, arthroscopically directed synovial biopsies were obtained before and at 24 hours after treatment, at disease relapse (4 patients), and after retreatment (1 patient). Membranes were stained by immunohistochemical techniques with monoclonal antibodies against TNFa, IL-8, IL-lp, and the IL-1 receptor antagonist protein (IL-1Ra).Results. MP therapy was associated with a rapid (within 24 hours) and substantial decrease in the expression of TNFa in the lining and sublining regions of the synovial membrane, as well as substantial decreases

21Canine leukocyte adhesion deficiency (CLAD) in Irish setters is caused by genetic 22 defects of leukocyte integrin CD18 leading to recurrent bacterial infections. We report 23 clinical features and analysis of neutrophil function from two mixed-breed canine 24 littermates (one female and one male dog) similar to CLAD. The symptoms of pyogenic 25 infection were first recognized at 3 months of age and since then the patients suffered 26 from recurrent bacterial infections. These clinical findings were strongly suggestive of 27 genetic phagocyte dysfunction. Neutrophil function tests revealed a marked reduction of 28 serum-opsonized zymosan-mediated superoxide production in the two littermates. 29Neutrophils of the male dog revealed impaired integrin-mediated adherence and 30 phagocytic activity, whereas ability of serum opsonization was normal. There was also a 31 profound decrease of surface expression of CD11b/CD18 and 2-integrin transcript 32 level, detected by real-time RT-PCR without missense mutations unlike CLAD. 33Immunoblot analysis indicated that protein expression of cytochrome b 558 component 34 gp91 phox , the cytosolic components p47 phox and p67 phox of NADPH oxidase components 35 increased profoundly in the male. Our study suggests that decreased transcriptional 36 levels of 2-integrin without mutations, lead to downregulation of surface expression, 37 resulting in multiple defects in adhesion-related neutrophil functions and consequently, 38 recurrent bacterial infections from puppyhood. 39 40
