BackgroundOne of the essential properties of the root canal sealers is the adhesion to root canal dentin and their higher bond strength decreases the microleakage. The aim of the present study was to compare the effect of Different Intracanal medicaments on the push out bond strength of AH26 and MTA Fillapex sealers.Material and MethodsA total of 104 one-rooted extracted human teeth were divided into 4 (n=26) experimental groups. After the cleaning and shaping, the root canals were filled with Ca(OH)2, triantibiotic paste (TAP), Metapex or 2% chlorhexidine gel for two weeks. Then, intracanal medicaments were rinsed away and the samples in the sub-groups were obturated with gutta-percha and AH26 or MTA Fillapex sealers. After two weeks incubation, 2-mm-thick middle section of each root was then subjected to push-out testing. Data were analyzed with two-way ANOVA and LSD test.ResultsWith all the intracanal medicaments, the overall mean of bond strength values were significantly higher with AH26 compared to MTA Fillapex (p<0.05). With the use of MTA Fillapex the maximum and minimum means of bond strength values were recorded with CHX and Metapex and for AH26 were recorded with Ca(OH)2 and chlorhexidine, respectively.ConclusionsThe bond strengths of sealers to dentin are under the influence of pre-treatment with intracanal medicaments. Under the limitations of the present study, the effect of TAP on the bond strength of endodontic sealers was not negative. Key words:AH26, medicament, MTA Fillapex, push-out bond.

Introduction Elimination of microbial flora in cases of immature permanent teeth with necrotic pulp is both key and a challenging goal for the long-term success of regenerative therapy. Recent research has focused on the development of cell-friendly intracanal drug delivery systems. This in vitro study aimed to investigate the antimicrobial action of three-dimensional (3D) tubular-shaped triple antibiotic-eluting nanofibrous constructs against a multispecies biofilm on human dentin. Methods Polydioxanone (PDS) polymer solutions, antibiotic-free or incorporated with metronidazole, ciprofloxacin, and minocycline were electrospun into 3D tubular-shaped constructs. A multispecies biofilm consisting of Actinomyces naeslundii, Streptococcus sanguinis, and Enterococcus faecalis, was forced inside the dentinal tubules via centrifugation in a dentin slice in vitro model. The infected specimens were exposed to two experimental groups; i.e., 3D tubular-shaped triple antibiotic-eluting constructs and TAP, and two control groups (7-day biofilm untreated and antibiotic-free 3D tubular-shaped constructs). Biofilm elimination was quantitatively analyzed by Confocal Laser Scanning Microscopy (CLSM). Results CLSM analysis showed a dense population of viable (green) bacteria adhered to dentin and penetrated into the dentinal tubules. Upon 3D tubular-shaped triple antibiotic-eluting nanofibrous construct exposure, nearly complete elimination of viable bacteria on the dentin surface and inside the dentinal tubules was shown by the CLSM images, which was similar (p<0.05) to the bacterial death promoted by the TAP group but significantly greater when compared to both the antibiotic-free 3D tubular-shaped constructs and the control (saline). Conclusion The proposed 3D tubular-shaped antibiotic-eluting construct showed pronounced antimicrobial effects against the multispecies biofilm tested and therefore holds significant clinical potential as a disinfection strategy prior to regenerative endodontics.

Köklerin apikal 2 mm kısmı uzaklaştırılmıştır ve unicore frezler ile apikalden koronal Öz Objective: Aim of this study was to compare the double (DAP) and triple (TAP) antibiotic paste removal efficiency of conventional syringe irrigation (CSI), EndoActivator (EA), passive ultrasonic irrigation (PUI) and XP-endo finisher (XPF) from simulated immature root canals. Materials and Methods: A total of 180 extracted mandibular canine roots were used. The apical 2 mm of the roots were removed, and Unicore drills were penetrated through the apical to the coronal direction approximately 1 mm to simulate an immature apex. Canals were prepared up to size 40 by using the Mtwo system. Roots were split longitudinally and then temporarily reassembled. The samples were randomly divided into 2 main groups as DAP and TAP. Five specimens for each main group were kept as negative control group. Then the pastes were placed with lentulo spiral. After 1 week five specimens for each main group were kept as positive control group. Then the remaining specimens in DAP and TAP groups were further assigned to 4 subgroups according to the removal technique (group 1: CSI; group 2: EA; group 3: PUI, group 4: XPF). The root halves were examined under a stereomicroscope at 10x magnification. Kruskal-Wallis H and Mann-Whitney U tests were used for statistical analyses. Results: TAP and DAP were more effectively removed by XPF and PUI compared to CSI and EA (p<0.05). XPF and PUI DAP/TAP removal effectiveness was statistically similar (p>0.05). Moreover, CSI and EA showed similar effectiveness for both paste groups (p>0.05). Regarding the type of antibiotic paste, no significant difference between TAP and DAP was found (p>0.05). Conclusion: The tested irrigation systems could not completely remove either TAP or DAP. The use of PUI or the XPF was more effective compared to the use of the EA and CSI.
