Tramadol is an opioid analgesic used to alleviate acute and chronic pain. Nigella sativa oil is one of the traditional remedies with antioxidant activity. This study was designed in order to investigate the ultrastructural alterations induced by tramadol in the rat cerebral cortex and to find out any possible protective effect of N. sativa oil against these alterations. Twenty-four male albino rats were assigned to three groups. Group I received intraperitoneal and oral normal saline for 30 days. Group II received intraperitoneal injections of tramadol 20 mg/kg/day, 40 mg/kg/day and 80 mg/kg/day on the first, second and third 10 days of the study, respectively. Group III received intraperitoneal tramadol similar to Group II and oral N. sativa oil at a dose of 4 ml/kg/day for 30 days. Specimens from the motor area were obtained and processed for transmission electron microscopy. In the tramadol-treated group, pyramidal and granular cells appeared shrunken and showed ultrastructural features of apoptosis such as nuclear membrane invaginations, chromatin margination, dilated rough endoplasmic reticulum, dilated Golgi saccules, and mitochondria with disintegrated cristae. The myelinated axons showed disorganization and splitting of the myelin sheath and contained vacuoles and abnormal mitochondria. Administration of N. sativa oil partially protected the cortical neurons and myelinated axons against tramadol-induced changes. In conclusion, N. sativa oil alleviates ultrastructural apoptotic changes induced by tramadol in the rat motor cerebral cortex.

Pesticides are major contaminants of our environment. Chlorpyrifos (CPF), a broad spectrum organophosphate insecticide, is one of the most used organophosphate pesticides in domestic and industrial applications all over the world. This work was undertaken to evaluate the protective effect of propolis and nigella sativa oil against the toxic effects of Chlorpyrifos on the liver and testes of adult male albino rats. Material and methods: Eighty adult male albino rats were used in this study. They were divided into 8 equal groups: Group I (negative control group); Group II (positive control group); Group III gavaged with propolis (400mg/Kg b. w.); Group IV gavaged with nigella sativa oil (100 mg/Kg b.w.); Group V with CPF 6.75 mg/ Kg b. w. / (1/20 of the oral LD50); while group VI gavaged with propolis and CPF, group VII gavaged with nigella sativa oil and CPF, and group VIII gavaged with both propolis and nigella sativa oil and CPF in the same previous doses. All treatments were given by oral gavage once daily for 12 weeks. Blood samples were obtained for assessment of total antioxidant capacity (TAC). Liver and testicular specimens were obtained for assessment of malondialdehyde (MDA) and histopathological examination by light and electron microscopes. Results: CPF induced chronic toxic effects in the liver and testes which were evidenced by significant increases in hepatic and testicular MDA and significant decrease in TAC serum levels. These results were confirmed by structural and ultra-structural alterations in liver and testes of CPF treated rats. Administration of propolis or nigella sativa alone or in combination ameliorated CPF induced toxic effects, but the most remarkable effects were observed in rats treated with combination of Propolis and nigella sativa. Conclusion: Combination of propolis and nigella sativa had marked protective effects against CPF-induced toxicity in liver and testes of adult albino rats.

Millions of patients who administer or abuse tramadol to relieve chronic pain may suffer from infertility. Thymoquinone (TQ) is a major active component of nigella sativa oil, which has antioxidant and anti-inflammatory effects. The present work aimed to clarify effects of tramadol and TQ on epididymis of adult male rabbits. Forty adult male rabbits aged 3 months were used for one month & divided equally into 4 groups: Group 1 as a control group; group 2 was treated with tramadol 30 mg/kg orally/day, group 3 was treated with TQ 20 mg/kg orally/day and group 4 as a protected group that was treated with TQ 20 mg/kg & tramadol 30 mg/kg orally/day. The epididymis was processed for histological and immunohistochemical (IHC) study for detection of epididymal structure and caspase-3 as an indicator of epididymal apoptotic changes. Statistical analysis of apoptotic index, area% of collagen and caspase-3 immunoexpression was carried out. Group 2 showed epithelial thinning of epididymal wall, degeneration and vacuolation of epithelial cells with disturbance of stromal collagen fibers. Also, a significant increase of apoptotic index, area % of collagen and caspase-3 cytoplasmic immunoexpression in epididymal epithelium was noticed in group 2. While, Group 3 and 4 showed improvement of the histological and IHC changes described in group 2. It is concluded that tramadol intake exerted toxic effects on epididymal structure while, TQ minimized these effects. Therefore, intake of TQ with tramadol is advised to protect the epididymis from the hazardous effects of tramadol abuse.
