Effect of Repeated Injection and Continuous Infusion of Omeprazole and Ranitidine on Intragastric Ph Over 72 Hours Figure 1

Abstract: Our study shows, for the first time, that omeprazole infusion was significantly superior to all other regimens by having a high median pH >6 on each day. The tolerance effect of ranitidine, however, led to a rapid loss of antisecretory activity on days 2 and 3, rendering it inappropriate for situations in which high intragastric pH-levels appear to be essential.

“…PPIs increase intragastric pH effectively and reduce rebleeding in patients with peptic ulcer bleeding; however, the optimal dose of PPI is unknown. With PPI use, intragastric pH increases >6.0 for 84–99% of the day, which is optimal for stability of clot and hemostasis [23]. In a recent study, Hung et al [17] reported that bolus PPI is more effective in preventing rebleeding than without treatment.…”

“…For example, an infusion of omeprazole (Prilosec) can keep the gastric pH above 6 for 72 hours without inducing tachyphylaxis. 20,21 Started after endoscopy. Randomized controlled trials have found proton pump inhibitors to be effective when given in high doses intravenously for 72 hours after successful endoscopic treatment of bleeding ulcers with high-risk endoscopic signs, such as active bleeding or nonbleeding visible vessels.…”

“…This attenuation of antisecretory activity has also been documented for other H 2 -receptor antagonists. 3,23) Netzer et al reported that tolerance to the antisecretory activity of ranitidine develops during infusion for 72-h periods leading to a loss of pH control. 3) In addition, Komazawa et al reported that tolerance occurs during 14 d of continuous famotidine administration.…”
