Effect of intraarterial papaverine or nimodipine on vessel diameter in patients with cerebral vasospasm after subarachnoid hemorrhage

Abstract: P and N seem to differ in the effect on cerebral diameter reduction in patients with vasospasm after SAH. The clinical implications remain to be established. A multimodal approach, perhaps combining different agents for intraarterial infusion in such patients, needs to be evaluated.

“…Angiographic resolution of diameter reduction and angiographically assessed capillary blood flow together differed not significantly between papaverine and nimodipine treated groups. Papaverine infusion dilated all angiographic demonstrable vessels while nimodipine infusion was ineffective in 16% of the patients [90]. Capillary flow on pre-and post-infusion angiographies was not different between these two groups.…”

“…Çoklu tedavileri gerektiren geçici etkisi ile beraber, papaverin kullanı-mının diğer bazı yan etkileri de mevcuttur. En ciddi ve hasar verebilen etkisi intrakraniyal basınçtaki artış (14,15,(22)(23)(24) , VS'ın kötüleşmesi, nörolojik bozulma ile gri madde hasarı (20,21,25) , beyin kök depresyonu (posterior sirkülasyonda kullanılması durumunda dikkat edilmelidir), nöbetler, kristal emboli oluşumuna ikincil olarak meydana gelen monoküler körlük ve geçici fokal nörolojik hasarlar olarak sıralanabilir (1,26) .…”

“…Papaverine is a short-acting vasodilator drug used most commonly in the treatment of arterial spasm and simulates the post-prandial state. 9,10 The 'normalization' of the pressure variables after intervention is unknown, but we would advocate a P d /P a ratio >0.90 and P a -P d (p) value of <20 mmHg, as evidenced in this case. This technique provided the necessary information to evaluate the hemodynamic significance of flow in the SMA after vasodilation before and after intervention.…”
