Abstract: Background-Native atherosclerosis and in-stent restenosis are focal and evolve independently. The endothelium controls local arterial responses by transduction of shear stress. Characterization of endothelial shear stress (ESS) may allow for prediction of progression of atherosclerosis and in-stent restenosis. Methods and Results-By using intracoronary ultrasound, biplane coronary angiography, and measurement of coronary blood flow, we represented the artery in accurate 3D space and determined detailed charact… Show more

“…Discussed here is vascular remodelling with a focus on the process of intima media thickening (IMT), an important manifestation of atherosclerosis in the carotid artery. Finally, the important contribution of genetic factors to vascular remodelling, and IMT in particular, has been confirmed as a result of recent human and animal studies (1)(2)(3)(4).…”

“…High shear stress was atheroprotective for the RCA in that there was no IMT similar to human coronary arteries (3). Genetic factors appeared to play a small role in the response to high shear stress because there were no significant differences among inbred mice strains.…”

“…It is defined as the product of the velocity gradient near the wall and the blood viscosity; this definition suggests that SS varies proportionally with flow. Low and oscillatory SS are characterized by low time-averaged values (less than 1.5 N/m 2 ) and significant variations in direction and magnitude over short distances, and constitute major determinants of the localization, development and progression of atherosclerosis (20)(21)(22)(23)(24). Through complex mechanotransduction processes similar to those initiated by TS, coronary endothelium responds to low and oscillatory SS by adopting a vasoconstrictive (ET-1), proinflammatory (adhesion molecules and cytokines), pro-oxidative, growth-promoting and prothrombotic phenotype, ultimately acquiring a predisposition to atherosclerosis (16,20,24).…”
