Effect of Dexamethasone and Fluticasone on Airway Hyperresponsiveness in Horses With Inflammatory Airway Disease

Abstract: BackgroundAirway hyperresponsiveness (AWHR), expressed as hypersensitivity (PC 75 RL) or hyperreactivity (slope of the histamine dose‐response curve), is a feature of inflammatory airway disease (IAD) or mild equine asthma in horses. Glucocorticoids are used empirically to treat IAD.ObjectivesTo determine whether dexamethasone (DEX) (0.05 mg/kg IM q24h) and inhaled fluticasone (FLUT) (3,000 μg q12h) administered by inhalation are effective in decreasing AWHR, lung inflammation, and clinical signs in horses wit… Show more

“…Horses with mild asthma have empirically been treated with glucocorticoids. Dexamethasone (0.05 mg/kg IM q24h) and inhaled fluticasone (3000 μg q12h) are effective at reducing hypersensitivity and hyperreactivity in horses with mild asthma . However, no improvement in bronchoalveolar lavage (BAL) neutrophil percentage was observed after short‐term administration of glucocorticoids, which is consistent with findings in severe asthma studies where the air quality was not improved .…”

“…In IAD, mixed/neutrophilic and eosinophilic inflammation may be present 91. Similar to human asthma, RAO and IAD respond to beta-adrenergic agonists in combination with inhaled or systemic corticosteroids 92,93. Pathologically, RAO is associated with airway remodeling, increased airway mucus and epithelial mucous metaplasia, ASM hypertrophy, and subepithelial collagen deposition.…”

“…1,3 Treatment of equine asthma is directed at decreasing inflammation of the lungs. 4 Dexamethasone can quickly and effectively improve lung function and, ultimately, clinical signs. 1 Corticosteroids are potent anti-inflammatory drugs that effectively reduce airway hypersensitivity and inflammation in horses with equine asthma.…”
