Ectopic ACTH Secretion Secondary to Metastatic Acinic Cell Carcinoma of the Parotid Gland: A Case Report and Review of Current Evidence for Systemic Therapy

Abstract: Acinic cell carcinoma is a rare, typically indolent, neoplasm that arises in the salivary glands. Metastatic disease is uncommon, occurring in around 10% of cases. We report the case of a 46-year-old male in whom the first sign of disseminated disease was increased skin pigmentation due to paraneoplastic Cushing’s syndrome. He underwent 3 cycles of chemotherapy with carboplatin and paclitaxel with no symptomatic improvement and a mixed response on imaging. There is no evidence that systemic therapy prolongs su… Show more

“…After craniocerebral injury, the humoral changes mainly include increased release of antidiuretic hormone, activation of the sympathetic adrenal system, and altered secretion of hypothalamic hormone [25]. e destruction of the blood-CSF barrier can increase the secretion of pituitary hormone ACTH and promote the adrenal expression and release of COR through the hypothalamic-pituitary-adrenal regulatory axis, which can improve the body's antistress [26]. In this process, the body can protect itself.…”

“…There is only one case of a primary acinic cell carcinoma secreting ACTH (Castro Oliveira et al 2019). In cases where long-term follow-up has been performed, patients suffered from disease complications and died despite being treated with chemotherapy and/or radiation therapy (Shenoy et al 2011;Dacruz et al 2016;Castro Oliveira et al 2019;Wade et al 2020;Burch et al 2021).…”

“…Metastasis tends to occur through hematogenous rather than lymphatic pathways, and commonly affects the lungs, bones, liver, and other visceral organs [1,[14][15][16]. Along with increasing the complexity of (or precluding) complete surgical management, metastatic ACC has rarely also been associated with paraneoplastic syndromes such as ectopic adrenocorticotropin hormone (ACTH) release [17,18] with significantly increased morbidity and mortality. Intracranial metastasis is thought to be exceedingly rare and only reported a few times in the medical literature (as presented in our Results section).…”
