Abstract: A 23-year-old man with Dyke-Davidoff-Masson syndrome (DDMS) was admitted to the hospital with increasing frequency of epileptic seizures. Physical examination revealed mental retardation, left facial asymmetry, and left-sided spastic hemiparesis. Dysdiadochokinesia on the left upper limb was detected, and there was no dysmetria. MRI confirmed the well-known radiological features of DDMS. PET/CT demonstrated cerebral and contralateral cerebellar hypometabolism. We present DDMS with crossed cerebellar diaschisis… Show more

“…Similar mass effect resulting in CCD has also been described in other brain tumors (including primary glioma), chronic subdural hematoma, congenital insults, intracranial infections, and various dementia subtypes. 6,[12][13][14][15] This case is a rare demonstration of PCNS thalamic lymphoma with ipsilateral cerebral hypoperfusion and contralateral CCD likely due to the compression effect of posterior limb of internal capsule and interruption of corticoponto-cerebellar tract.…”

“…Thus, the age of presentation depends on the time of vascular occlusion of the middle cerebral vascular territory. This injury to the brain leads to cerebral hemiatrophy, which is associated with hypoperfusion and decreased metabolic activity of the affected cerebrum67. The cerebral atrophy is followed by compensatory changes in the adjacent areas only if brain damage occurs before three years of age8.…”

“…In congenital DDMS, ipsilateral shift of midline structures with calvarial changes is noted, while ipsilateral prominent sulcal spaces are seen if the insult occurs after sulcation is complete [1,2]. Crossed cerebrocerebellar diaschisis, ipsilateral basal ganglia and thalamus/ mesencephalic hypoplasia, secondary to wallerian/transneuronal degeneration, have been reported in DDMS [3,4]. Shen, et al have described three radiological patterns, based on cortical/subcortical atrophy, presence of porencephalic cyst, and chronic infarction/ necrosis in the middle cerebral artery territory [5].…”
