Durability of down-sized homografts for the reconstruction of the right ventricular outflow tract

Abstract: Down-sized homografts demonstrated a durability similar to that of small-sized homografts. Therefore, down-sizing adult-sized homografts by creating a bicuspid valve to fit into the corresponding RVOT in children with congenital heart defects is an excellent method to overcome the shortage of small-sized homografts.

“…Even homografts, which are widely used in western clinical centers but cannot be obtained in China due to legal problems and regional reasons, show a decay rate of 25% in 5 years and 45% in 10 years . Moreover, age is an important factor for biological valve failure . In short, the long‐term availability and durability of these valvar materials are not satisfactory, especially for younger children.…”

“…The down-sized technique was first described by Michler et al in 1994, obtaining a bicuspid valved conduit by removing one of the three valve leaflets and the attached wall ( 8 ). The largest study cohort of down-sized homograft conduits came from the German Heart Centre, Munich ( 9 ). Eighty-two patients receiving a down-sized homograft were compared to 70 patients receiving normal small-sized homograft.…”

“…One way to help deal with the shortage of small-sized conduits is the down-sized technique (8). We were pleased to find that this technique does not affect the the function and durability of the conduits (9,10). There is no doubt that the bovine jugular vein conduit (BJVC), usually known as Contegra R conduit (Medtronic Inc., Minneapolis, Minnesota, USA), has achieved great success.…”

“…This is the reason, why some surgeons started to "bicuspidalize" or "downsize" homografts, removing a cusp and sewing it back together to have a smaller-sized bicuspid homograft (Figures 1,2). This surgical method arose out of the need for small-sized homografts and is widely used in centres with access to homografts (4,5).…”
