Treatment guidelines for acute otitis media and otitis media with effusion have been discussed for many years. Children referred to otolaryngologists often have been treated adequately before and still continue to suffer from ear infections. They are called ‘otitis-prone’ and need to be identified and treated accordingly. Systematically evaluating the known risk factors and ranking the otoscopic findings as well as the hearing tests can help to recognize ‘high-risk’ children early on. These patients will benefit from a more offensive therapy, both medically as well as surgically, whereas ‘low-risk’ patients can be treated with a ‘watchful waiting’ policy.
