Distant metastases from head and neck squamous cell carcinomas

Abstract: Distant metastases (DMs) occurred in 83 (11.4%) of 727 retrospectively studied head and neck cancer patients. Primary tumor location and initial treatment did not influence DM development; larger primaries (P < .04) or more extensive neck disease (P < .007) more often caused DMs. Initial diagnosis to DMs averaged 11.7 months (range, 0 to 60 months), with 84% diagnosed within 24 months. With the exception of laryngeal primaries, no facet of tumor, host, or initial treatment influenced where or how rapidly DMs d… Show more

“…Mutations were found in either one or both tumors in 15 patients, which allowed differentiation between SPT and DM. Nine patients were classified as SPT (patients [1][2][3][4][5][6][7][8][9] and six as DM (patients 10-15). In patients 1, 2, 5, 6, 7, and 9, we found a TP53 mutation in the HNSCC and wild-type TP53 in the lung carcinoma.…”

“…SPT most frequently occur in the head and neck region, with the lungs as the second most common site (2,3). The lungs are also the most common site of DM in patients with head and neck squamous cell carcinoma (HNSCC), causing a problem in differential diagnosis (4). Early detection and treatment of small size primary lung carcinoma improves survival (5,6).…”

“…The four criteria used to determine suitability for surgical resection of pulmonary metastases were [1] presence of metastasis from a malignant tumor in the head or neck (excepting the thyroid), [2] complete response to radical therapy for the primary lesion and sustained complete remission of cancer thereafter, [3] no other distant metastases, and [4] complete pulmonary resection not contraindicated. We retrospectively reviewed treatment outcomes for these 16 cases of resected pulmonary metastases and attempted to identify clinicopathologic factors associated with prognosis.…”

“…Nevertheless, despite adequate local control, distant metastases frequently develop and are associated with unfavorable outcomes [1,2]. The most common metastasis of head and neck carcinoma is hematogenous metastasis to the lungs; however, there are few data on survival rates after metastasectomy, and treatment in most reported cases did not include surgical resection of distant metastases.…”
