Background Quality of life (QOL) is a multidimensional concept that is used to describe the individuals' perceptions, satisfaction, and evaluation of different areas of their own lives, such as physical health and functioning, psychological and emotional wellbeing, social roles, and relationships. Few studies have addressed the effect of rheumatoid disease on the QOL in the Egyptian community. Results Functional disability based on health assessment questionnaire disability index was significantly increased with the increase in disease activity based on disease activity score-28, where all patients on remission and 56% of those with low disease activity had mild functional disability. Approximately 89% of patients with low disease activity had mild anxiety based on Hamilton anxiety scale, and the majority (75%) of those patients with moderate disease activity and 83.3% of patients on remission had mild anxiety. The majority (85.7%) of those with high disease activity were depressed based on Zung self-rating depression score. Disease activity score-28 score had a positive significant correlation with health assessment questionnaire disability index (P=0.01, r=0.40), Hamilton anxiety scale-A (P=0.01, r=0.46), Zung score (P=0.01, r=0.46), and rheumatoid arthritis QOL (r=0.70, P=0.00), and negative correlation with female sexual index (r=−0.80, P=0.00), all domains of SF-36, and all five domains of WHOQOL. Conclusion This study confirms that rheumatoid arthritis causes impairment of all aspects of QOL (limitation of physical function, physical disability, and pain), mental health disorders (anxiety and depression), and social, environmental, and also sexual dysfunction.

Pendahuluan. Artritis Reumatoid (AR) merupakan penyakit kronik sistemik yang sering disertai dengan depresi pada 20-30% pasiennya. Derajat aktivitas penyakit AR dinilai dapat memengaruhi terjadinya depresi. Penelitian ini dilakukan untuk mengetahui proporsi depresi pada pasien AR dan hubungan antara derajat aktivitas penyakit dengan depresi pada pasien AR. Metode. Penelitian ini merupakan studi potong lintang yang dilakukan dengan memeriksa pasien AR di Poliklinik Reumatologi Departemen Ilmu Penyakit Dalam RS Cipto Mangunkusumo Jakarta yang memenuhi kriteria inklusi secara konsekutif pada bulan Januari sampai Maret 2017. Derajat aktivitas penyakit AR dinilai dengan menggunakan Disease Activity Score-28 (DAS 28) dan depresi dinilai dengan kuesioner Back Depression Inventory (BDI). Analasis statistik dilakukan dengan menggunakan dengan program SPSS versi 20.0. Hasil. Dari 145 subjek yang ikut dalam penelitian, sebanyak 90,3% di antaranya adalah wanita (131 orang). Median usia subjek adalah 55 tahun (rentang 19-83 tahun). Sebanyak 45 subjek (31%) memiliki masalah psikososial (stresor). Hasil analisis menunjukkan bahwa proporsi depresi pada pasien AR sebesar 35,9% (IK 95%=30-42%). Derajat aktivitas penyakit subjek yang diukur dengan DAS 28 menunjukkan bahwa proporsi subjek dengan derajat aktivitas AR ringan, sedang, dan berat secara berturut-turut yaitu 24 (82,8%), 52 (66,7%), dan 4 (23,5%). Hasil analisis chi-square menunjukkan bahwa terdapat hubungan yang bermakna secara statistik antara derajat aktivitas penyakit dengan depresi pada pasien AR (p = 0,001). Simpulan. Proporsi kejadian depresi pada pasien AR di RSCM adalah sebesar 35,9%. Derajat aktivitas penyakit memiliki hubungan yang bermakna dengan depresi pada pasien AR.

Background: Depression affects 14.8% – 38.8% of patients with rheumatoid arthritis (RA) in developed countries. The prevalence and risk factors for depression in patients with RA in sub-Saharan Africa is not well established.Aim: To determine the prevalence of depressive symptoms in patients with RA.Setting: Public sector regional hospital in South Africa.Methods: A cross-sectional descriptive study was undertaken with 110 adult RA patients. A structured socio-demographic and clinical questionnaire, the modified health assessment questionnaire (mHAQ), the simplified disease activity index (SDAI) for RA, the patient health questionnaire (PHQ-9), and the Household Food Insecurity Access scale (HFIAS) for nutritional status, were used. Correlates of depressive symptomatology in participants with RA were identified using t-tests and regression analyses.Results: Most of the participants were women (90.9%), 67% had moderate to severe RA disease on the SDAI score, 92.7% reported functional disability (HAQ score of ≥ 1), and 87.2% reported mild to severe depressive symptoms. Unemployment (p 0.01), severe food insecurity (p 0.01) and functional disability (p = 0.02), were significantly associated with the depressive symptoms, but not with disease activity (p = 0.8) or inflammatory markers (p = 0.63). Unemployment (adjusted β = −5.07, p 0.01) and severe food insecurity (adjusted β = −4.47, p 0.01) were significantly associated with depressive symptoms, based on the adjusted regression model.Conclusion: As RA effects functional status, with the impact of the resulting unemployment and food insecurity being associated with depression, affected people should be screened for depression and managed using a multidisciplinary approach, especially considering the role of social determinants in RA patients with depression.
