This study aimed to explore the relationship between individual social capital and functional ability, with a focus on whether there is an interactive relationship that exists among social capital related to functional ability among older people in Anhui province, China. We conducted a cross-sectional study with a multi-stage stratified cluster random sampling method from July to September 2017. Data were collected through questionnaire including demographic characteristics, individual social capital status, and functional capability status. Binary logistic regression analysis model and classification and regression tree model (CART) were utilized. Overall, this study included 1810 elderly people, 43% of whom had functional disability. After the adjustment, subjects with lower social participation (AOR = 1.60; 95% CI: 1.26–2.03) and lower social connection (AOR = 1.74; 95% CI: 1.34–2.25) had an increased risk of functional disability. However, social support (AOR = 0.73; 95% CI: 0.57–0.94) was inversely related to functional ability. We also observed interactive relationship of social capital associated with functional ability, which indicated that special attention and efforts should be paid to older adults with less educational attainment, with multimorbidity, with advanced age, and with lower level of social participation, cohesion for the purpose of maintaining sound functional ability. Our findings may be of salient relevance for devising more targeted and effective interventions to prevent the onset of functional limitations among community-dwelling older adults.

RESUMO: Introdução: A funcionalidade no envelhecimento está associada à autonomia e independência das pessoas idosas. Objetivo: Identificar e hierarquizar as dificuldades referidas no desempenho das atividades de vida diária de idosos. Método: Estudo transversal e descritivo, de base domiciliar, que utilizou a base de dados do Estudo SABE (Saúde, Bem-Estar e Envelhecimento) nos anos de 2000, 2006 e 2010. A funcionalidade foi avaliada por meio do relato de dificuldade no desempenho das atividades básicas (ABVD) e instrumentais de vida diária (AIVD). A hierarquização do comprometimento funcional foi avaliada pelo escalonamento de Guttman. Resultados: A prevalência de dificuldade referida no desempenho de uma ou mais AIVDs foi de 35,4; 45,8 e 41,0% e para as ABVDs foi de 16,3; 13,3 e 17,5%; respectivamente nos anos de 2000, 2006 e 2010. Em dez anos de acompanhamento observa-se, entre as mulheres, variabilidade na prevalência de 42,3 a 54,6% de comprometimento nas AIVDs e de 17,0 a 20,4% nas ABVDs e, entre os homens, de 25,6 a 33,1% em AIVDs e de 8,0 a 13,7% em ABVDs. Nas três ondas, as atividades com maior relato de dificuldade foram utilizar transporte, realizar tarefas pesadas e cuidar das finanças, enquanto o ato de comer foi a menos prevalente. Conclusão: Houve aumento na prevalência de comprometimento funcional no período de dez anos, com maior variabilidade entre as mulheres e com o avançar da idade. Esses resultados contribuem para o planejamento dos serviços e a distribuição adequada dos recursos existentes por desvelar as necessidades e os cuidados necessários.

Abstract: Older adults were mostly disabled in physical related activities, whereas disability in more cognitive related activities was less often experienced. The impact of multimorbidity on disability in each activity was substantial, while education was not.
