Direct Round Window Application of Gentamicin with Varying Delivery Vehicles: A Comparison of Ototoxicity

Abstract: The fibrin/Gelfoam vehicle was the only delivery system to produce reliable damage. Ototoxic damage depended on the vehicle. We found no evidence for selective vestibular ototoxicity with gentamicin.

“…Other studies have reported the effectiveness of distinct biodegradable hydrogels for IT delivery of drugs such as gentamicin [Kelly et al, 1999;Sheppard et al, 2004], neurotrophins [Endo et al, 2005] and beclomethasone [Arnold et al, 2005]. In our study, an HAbased hydrogel containing thiol-modified HA and gelatin at a ratio of 9: 1 was used to deliver dexamethasone to the inner ear.…”

“…However, the efficiency of HA as a carrier agent for IT delivery appears to be dependent on the specific drug being delivered and on the gel composition [Kelly et al, 1999;Chandrasekhar et al, 2000;Sheppard et al, 2004;Endo et al, 2005;Tsuyoshi et al, 2005;Yildirim et al, 2005;Coleman et al, 2007;James et al, 2008;Saber et al, 2009]. In a guinea pig model, 1 h after IT treatment, perilymph levels of dexamethasone were 20% higher when used in the presence of HA (9.5 mg/ml) compared with dexamethasone alone [Chan-drasekhar et al, 2000].…”

“…The total volume injected is likely to be a major factor in such observations. Hyaluronate gels applied to the middle ear space are reported to have no toxic effects (Engstrom et al, 1987,Laurent et al, 1992,Sheppard et al, 2004. Studies investigating the functional consequences of the RW injection protocol reported here are now ongoing.…”

“…This may be achieved by injecting viscous gel formulations, or by placing wicks, microcatheters [35] into, or close to, the round window niche. Gel-based formulations include hyaluronates [36], collagens [37], chitosans [38], fibrins [39], starch, celluloses, 6 elatin [40], poloxamers [41], and many others. Viscosity that is too high may also result in the formation of an air bubble on the RWM, thus preventing effective diffusion, or may temporarily impair the ossicular mobility.…”
