Abstract: BackgroundThere is no consensus on which lung-protective strategies should be used in cardiac surgery patients. Sparse and small randomized clinical and animal trials suggest that maintaining mechanical ventilation during cardiopulmonary bypass is protective on the lungs. Unfortunately, such evidence is weak as it comes from surrogate and minor clinical endpoints mainly limited to elective coronary surgery. According to the available data in the academic literature, an unquestionable standardized strategy of l… Show more

“…Several previous studies aimed to prove the beneficial effect of protective ventilation in cardiac surgery [ 16 ]. However, most trials were insufficiently powered or biased, leading to high heterogeneity and lack of conclusive results [ 13 , 19 31 ]. The CPBVENT trial (NCT02090205) aims to answer a similar question.…”

“…This explains why the number of patients to be included in CPBVENT is smaller (n = 720 vs. n = 1500 in our study). Moreover, CPBVENT compares three ventilation strategies (no ventilation, continuous positive airway pressure (CPAP) and LTV ventilation), which may decrease the power of the study [ 13 ]. Finally, the inclusion criteria are stricter, decreasing the possibility to generalize the results.…”

“…The mechanical VENTilation during CardioPulmonary Bypass (CPBVENT) trial will be enrolling patients to compare outcomes among 3 different ventilation strategies on a prospective basis. 10 The results of this study will clarify some of the questions that Wang et al were unable to elucidate fully. Additional studies also may be needed to elucidate the impact of prebypass and postbypass ventilation strategy and fraction of inspired oxygen on perioperative and postoperative outcomes in this patient population.…”
