Objectives. The present study was conducted to categorise the morphological features on bone marrow aspirate cytology into common, uncommon, and atypical features in a nonendemic region which would be helpful in clinching an early and correct diagnosis especially in clinically unsuspected cases. Methods. The morphological features on bone marrow were categorized into common, uncommon, and atypical in cases of leishmaniasis from non endemic region. Results. Out of total 27 cases, 77.7% were residents of places at the height of 500 m or above and fever was the most common presentation followed by hepatosplenomegaly. Plasmacytosis, hemophagocytosis were the common cytological features while dysmyelopoiesis, presence of leishmania bodies in nonhistiocytic cells, and granuloma with necrosis were uncommon features. Aggregates of LD bodies in form of ring, floret, or strap shapes along with giant cells constitute the atypical morphological features. Conclusion. The knowledge of common, uncommon, and atypical features on bone marrow aspirate cytology is helpful in clinching an early and correct diagnosis of leishmaniasis especially in non endemic areas where clinical suspicion is low. These features will guide the pathologist for vigilant search of LD bodies in the marrow for definite diagnosis and thus will also be helpful in preventing unnecessary workups.

Introduction-Leishmaniasis which is caused by Leishmania donovani is considered to be a disease of lowland as climate and geographical factors play an important role in its distribution. However, recently few studies have reported leishmaniasis at higher altitude of Himalayan and sub Himalayan region of India. The present study was therefore conducted to study the clinico-hematological profile and the various morphological features on bone marrow examination associated with leishmaniasis in the non-endemic sub Himalayan region of India. Material and Methods-The study included all the cases of leishmaniasis which were diagnosed on bone marrow examination over a period of one and half year in a medical institute situated in Uttarakhand state of India. Results-The study included total 10 cases of visceral leishmaniasis diagnosed on bone marrow aspirate with fever (10 cases) and hepato-splenomegaly (7 cases) being most common clinical presentation and pancytopenia (9 cases) being the most common haematological finding. Bone marrow aspirate cytology showed that plasmacytosis (9 cases) and hemophagocytosis (8 cases) were the most common features. One case was clinically suspected as acute leukemia which turned out to be a case of leishmaniasis on bone marrow examination. Conclusion-Leishmaniasis is emerging as an important major parasitic infection in the nonendemic Himalayan and sub Himalayan region of India. Fever, splenomegaly and pancytopenia are important clinical and peripheral blood examination features associated with this disease. Hemophagocytosis and plasmacytosis on bone marrow examination should prompt for vigilant search for LD bodies in bone marrow to avoid missing of diagnosis even in unsuspected cases.

Objetivo: Conocer el origen del brote de Leishmaniasis producido en 2009-2014 en la Comunidad Autónoma de Madrid (C.A.M.) y dar a conocer las pautas de prevención. Metodología: Se realizó una revisión bibliográfica a través de varias bases de datos utilizando diferentes descriptores relacionados con el brote de Leishmaniasis en la C.A.M. acontecido entre 2009 -2014. Resultados: Se obtuvieron un total de 177 estudios sobre Leishmaniasis, de los cuales 29 estudios analizan aspectos relevantes sobre la Leishmaniasis. Los estudios afirman que la modificación por el ser humano de los grandes paisajes verdes y el avance en la construcción de nuevas autovías y redes de cercanías junto con las condiciones climáticas podrían haber sido relevantes para explicar el comportamiento espacial del brote, contribuyendo a la creación de un ecosistema favorable para la proliferación y estabilización de un nuevo ciclo urbano de Leishmaniasis. La única medida de prevención eficaz es actuar de forma adecuada para evitar la picadura del mosquito. Conclusiones: Cabe destacar que para la creación de un nuevo ciclo urbano de Leishmaniasis podrían haber contribuido factores relevantes como el crecimiento poblacional, la presencia de grandes espacios verdes de reciente construcción periurbana y la construcción de extensas infraestructuras de ferrocarril y carreteras. Aunque se ha trabajado de forma activa y conjunta desde las Autoridades Sanitarias y han disminuido el número de casos, el problema no se ha erradicado, por tanto juega un papel muy importante la difusión y aplicación de las medidas preventivas, siendo esencial que la población conozca cómo debe actuar.
