DIAGNOSIS OF ENDOCRINE DISEASE: Expanding the cause of hypopituitarism

Abstract: Hypopituitarism is defined as one or more pituitary hormone deficits due to a lesion in the hypothalamic-pituitary region. By far, the most common cause of hypopituitarism associated with a sellar mass is a pituitary adenoma. A high index of suspicion is required for diagnosing hypopituitarism in several other conditions such as other massess in the sellar and parasellar region, brain damage caused by radiation and by traumatic brain injury, vascular lesions, infiltrative/immunological/inflammatory diseases (l… Show more

“…Pituitary abscesses and the infection of the central nervous system of various etiologies, tuberculosis-TBC, and other bacterial, viral, and fungal infections can affect hypothalamo/pituitary axis and disrupt its function [24, 25]. The importance of early diagnosis of these rare alternative etiologies of hypopituitarism has been emphasized [26, 27]. Unrecognized hypopituitarism may be misdiagnosed as persistent neurologic sequel or cognitive impairment in TBI; pituitary abscess can be misinterpreted as a pituitary adenoma, while pituitary insufficiency following meningoencephalitis may be misunderstood as postencephalitic syndrome.…”

“…The typical tumour-related pituitary mass effects are: headache, visual field anomalies, diabetes insipidus and hypopituitarism that are registered in macroadenomas regardless their endocrine activity (24). Hypothyroidism is manifested with dry skin, fragile hair, cold intolerance; secondary chronic adrenal insufficiency exhibits pale skin (opposite to hyper-pigmentation due to high ACTH in primary adrenal insufficiency) (25).…”
