Diagnosis and treatment of the neutrophilic dermatoses (pyoderma gangrenosum, Sweet's syndrome)

Abstract: Neutrophilic dermatoses include a spectrum of disorders with similar histologic appearance and pathologic processes. Clinically, however, they have different physical manifestations and associations. This group includes two diseases for which dermatologists are commonly consulted in the hospital, namely pyoderma gangrenosum and acute febrile neutrophilic dermatosis, or Sweet's syndrome. Evaluation is challenging, and many therapeutic approaches have been described for both. The previously reported diagnostic c… Show more

“…In addition to the two major criteria (abrupt onset of tender or painful erythematous or violaceous plaques or nodules and neutrophilic infiltration of the dermis without leukocytoclastic vasculitis), he had three minor criteria (elevation of the CRP level, leukocytosis and preceding slight fever) [2]. Like our case report, cases of SS with recurrent oral ulcerations were reported [1,[4][5][6][7].…”

“…Neutrophilic dermatoses, including Sweet's syndrome (SS), pyoderma gangrenosum (PG), subcorneal pustular dermatosis (Sneddon-Wilkinson disease (SW)) and rheumatoid neutrophilic dermatitis, are inflammatory conditions of the skin often associated with underlying systemic disease [2]. Some deserve great attention due to major symptoms and the concept of reactive disease or for being a marker for other pathologies, especially neoplastic diseases.…”

“…Muchas veces estas patologías se relacionan con una respuesta inmune inadecuada la cual puede desencadenar PG. Su resolución puede conducir también a la cura de PG, aunque algunas veces esta se comporta en forma independiente a la enfermedad asociada 19 . En el tratamiento sistémico, uno de los esteroides más usados es la prednisona en dosis de 1-2 mg/ Kg/dia, debido a su rápida respuesta (2-3 días) 1 .…”

“…16 Although no randomized controlled trials have been performed, systemic corticosteroids have been the mainstay of treatment and typically, a rapid response is observed within days to weeks. In combination with steroids or in resistant cases, other immunosuppressant agents may be used, such as cyclosporine A, azathioprine, methotrexate, or mycofenolatemofetil.…”
