Хлопчик О., народжений на 38-му тижні гестації від V вагітності, III пологів (у матері було два викид-ні). Маса при народженні 2300 г. Стан після наро-дження задовільний.Дитина надійшла у відділення патології ново-народжених комунальної міської дитячої клінічної лікарні м. Львова на 26-й день життя.Анамнез захворювання: зі слів матері, хлопчик хворіє 5-ту добу. Захворювання почалося з нежиті, на 2-гу добу приєдналися кашель і задишка.Âèïàäîê ³ç ïðàêòèêè / Case Report ® Стан при надходженні тяжкий. Огляд. Наявний періоральний ціаноз. Тургор тканин задовільний. Дихання носом утруднене че-рез масивний набряк носових ходів. При аускульта-ції в легенях жорстке дихання, з обох сторін вологі хрипи, що дуже повільно зменшувалися впродовж лікування, та спостерігався короткочасний ефект після інгаляційно-дренажної терапії. Background: Anomalies of branchial arches are uncommon anomalies of embryonic development and may present as cysts, sinus tracts, fistulae or cartilaginous remnants. We describe our experience with 30 children with branchial cleft anomalies.Methods: Case records of all patients were retrospectively reviewed and analysed with respect to age, sex, clinical presentation, duration of symptoms, investigations, management and follow up. All patients underwent complete excision of the tract/cyst.Results: There were 15 males and 15 females (ratio of 1:1). The average age of presentation was 5 years. Majority of the patients presented with discharging sinuses (n=26). Twenty-six patients had branchial sinuses, three patients had branchial cysts and one had pyriform fistula. The anomalies were lateralized to left side in 17 patients (56.66%), right side in 11 patients (36.66%) and bilateral in 2 patients (0.066%). Complete excision was done in all patients. Four patients presented with abscesses and required incision and drainage. Definitive surgery was done after 6 weeks. There were no complications.Conclusions: Abnormal development of branchial apparatus lead to formation of different anomalies which remain asymptomatic and present later in life as cysts, sinuses or fistulae in neck. Diagnosis is mostly clinical and complete excision provides definitive cure.

Detection and significance of main anchoring villus in early pregnancy

Objective: To find and explore the relationship among the length of the embryo's main anchoring villus and the week of gestation, embryonic crown-rump length (CRL), and maternal blood human chorionic gonadotropin (hCG) during early pregnancy using ultrasound.Methods: A total of 108 women with singleton pregnancies underwent ultrasound examination of the fetus during the early months of their pregnancy (5 +5 -13 +6 weeks of pregnancy). The main anchoring villus was detected using HD-Flow and SlowflowHD mode ultrasound. The detection rate and length of the main anchoring villus were recorded. The correlations among main anchoring villus with gestational weeks, CRL, and blood hCG were obtained. Results:The main anchoring villus was detected in all women (108/108). It could be found throughout the early gestational weeks from as early as 4 +6 weeks. The length of the main anchoring villus increased with gestational age, and was positively correlated with CRL and the logarithm of hCG (P < 0.05). Conclusion:Our results showed that the detection rate of the main anchoring villus was 100% and the length of the main anchoring villus was correlated with gestational week, CRL, and blood hCG. The main anchoring villus could represent where the embryo implanted.
