Development of a screening tool predicting the transition from acute to chronic low back pain for patients in a GP setting: Protocol of a multinational prospective cohort study

Abstract: Background: Low back pain (LBP) is by far the most prevalent and costly musculoskeletal problem in our society today. Following the recommendations of the Multinational Musculoskeletal Inception Cohort Study (MMICS) Statement, our study aims to define outcome assessment tools for patients with acute LBP and the time point at which chronic LBP becomes manifest and to identify patient characteristics which increase the risk of chronicity.

“…To optimise the decision-making process of physicians these factors should be included in future screening instruments for the identification of prognostic factors at an early stage. Furthermore, psychological and occupational factors should be part of a minimum core set in future prospective studies as referred to in an ongoing study of the authors' looking into predictors of the transition from acute to chronic LBP, including the impact of different health care systems [13].…”

“…Recurrent LBP was defined according to Stanton et al as LBP with a minimum of 30 painfree days between the last two LBP episodes and a pain score on the Visual Analogue Scale (VAS) higher than 20 out of a maximum of 100 points 22) . The protocol of our study has been published previously 20) .…”

“…We performed a prospective cohort study investigating 310 patients presenting to a health practitioner with a new episode of acute/ subacute LBP, or with recurrent LBP 20) . Our study was conducted according to the recommendations of the Declaration of Helsinki (2008) and was approved by the local Lower South Regional Ethics Committee (LRS/08/03/008).…”

“…These factors, mainly psychological and occupational, have proved more reliable as predictors of chronic low back pain. These indicators, identified by Melloh et al (2008) 27 ( Figure 1) should be taken into account by professional therapists and clinicians working with these patients and include them as areas that should be involved in the treatment plan.…”
