Background and objectivesPatient knowledge of lung function (ie, forced expiratory volume in 1 s [FEV1]% predicted) and the intended benefits of their prescribed pulmonary medications might play an important role in medication adherence, but this relationship has not been examined previously in patients with cystic fibrosis (CF).MethodsAll patients diagnosed with CF and without prior lung transplantation were invited to complete knowledge and self-reported medication adherence questionnaires during routine outpatient visits to the Adult CF Clinic, St Paul’s Hospital, Vancouver, Canada from June 2013 to August 2014.ResultsA total of 142 out of 167 (85%) consecutive adults attending CF clinic completed patient knowledge and medication adherence survey questionnaires. Sixty-four percent of the patients recalled their last FEV1% predicted value within 5%, and 70% knew the intended benefits of all their prescribed medications. Self-reported adherence rates were highest for inhaled antibiotics (81%), azithromycin (87%), and dornase alpha (76%) and lowest for hypertonic saline (47%). Individuals who knew their FEV1% predicted value within 5% were more likely to self-report adherence to dornase alpha (84% vs 62%, P=0.06) and inhaled antibiotics (88% vs 64%, P=0.06) compared to those who did not, but these associations were not statistically significant. There were no significant associations observed between patient knowledge of intended medication benefits and self-reported medication adherence.ConclusionContrary to our hypothesis, disease- and treatment-related knowledge was not associated with self-reported medication adherence. This suggests other barriers to medication adherence should be targeted in future studies aiming to improve medication adherence in adults with CF.

ÖZET Amaç: Bu kesitsel çalışmanın amacı ebeveynlerin pediatrik uyku apnesi ile ilgili bilgilerini ve ortodontistin bu hastalığın teşhis ve tedavisindeki yeri ile ilgili fikirlerini değerlendirme-ktir. Gereç ve Yöntem: Pediatrik uyku apnesi hakkındaki farkın-dalık ve görüşleri ölçmek üzere tasarlanmış bir anket, çocuklarının ortodontik tedavisi için bir üniversite ortodonti kliniğine başvuran uygun örneklem grubu ebeveynlere uygulandı. Elde edilen veriler ortalama ve sıklıkları içeren tanım-layıcı istatistikler ile analiz edildi. Demografik değişkenler ile sorgulanan bilgi maddeleri arasındaki korelasyonlar Pearson ki-kare testi kullanılarak incelendi. Bulgular: Anket toplam 260 ebeveyn tarafından cevaplandı. (Ortalama yaş: 40,7±6,6 yıl, 169 kadın, 91 erkek). Ebeveynlerin çoğu, diş hekimleri ve ortodontistlerin tıbbi hastalık riskleri yönünden çocuklarında değerlendirme yapmasının çok önemli olduğu görüşündeydi (sırasıyla %76,5 ve %76,2). Ebeveynler aynı zamanda ortodontistin çocuklarını uyku apnesi riski yönünden değerlendirmesini isteyeceklerini (%92,7) ve eğer ortodontist yönlendirirse uyku apnesi açısından değerlendirilmek üzere çocuklarını doktora götüreceklerini (%97,5) belirttiler. Çoğu ebeveyn pediatrik uyku apnesi hakkında temel düzeyde bir bilgiye sahip olmasına rağmen, bu hastalığın etyolojisi, semptomları, teşhisi, sonuçları ve tedavileri hakkında bilgileri yoktu. Artmış eğitim seviyesi, pediatrik uyku apnesi hakkındaki bilgi düzeyi ile anlamlı düzeyde ilişkiliydi (p<0,05). Low adherence to cystic fibrosis (CF) treatment is associated with poor health outcomes, while knowledge of the disease and medication regimen can positively influence adherence. This study’s purpose was to develop and validate a questionnaire to help determine CF medication knowledge of pediatric patients and caregivers. Our questionnaire had 37 items: 22 selected-response and 15 open-response questions. We used validation processes from the Standards for Educational and Psychological Testing. CF experts analyzed validity evidence based on content. Then, the questionnaire was field tested with 17 pediatric patients and 18 caregivers. The correlation between age and medication knowledge was medium (r = .33), but was not significant (P = .189). Cronbach’s α for the overall test was .84. Participants thought the questionnaire was important and suitable, with a few minor suggestions to improve wording. Strong validity evidence indicates the questionnaire could be used to assess medication knowledge and allow more personalized education to improve adherence.
