Abstract: The rosette structure is a dermoscopic sign visible under polarized light, characterized by 4 white points arranged as a 4-leaf clover. It has been mainly described in facial sun-damaged skin and actinic keratosis, although it has also been found in squamous and basal cell carcinomas, and in 2 cases of hypomelanotic melanomas. We describe 2 different cases of pigmented incipient melanomas with the presence of multiple rosettes and shiny white structures on dermoscopy. In the reflectance confocal microscope the… Show more

“…White shiny lines/areas and rainbow pattern probably correspond to the optical effect of the PL interacting with fibrous and vascular structures respectively. Conversely, rosettes may be due to a PL diffraction phenomenon occurring at the adnexal openings . In this study, we found that shiny white lines/areas and rainbow pattern were more common in palpable BCCs (plaque and nBCCs), whereas rosettes were more frequent in non‐palpable sBCCs.…”

“…The expert panel also considered other important decisive factors such as adequacy, reproducibility, significance, and lack of redundancy. With regard to significance it is important to note that we included terms even if they are not highly specific, such as “rosettes” 73,100103 or “rainbow pattern,” 33,104106 because they describe common features or patterns. Notably, terms that have not been included in the dictionary may still be useful.…”

“…These have been mainly described in actinic keratosis, and squamous and basal cell carcinomas; a few cases of melanoma can show rosette structure as well. On histological examination, rosettes correspond to focal hyperkeratosis alternating with a normal‐appearing horny layer and with keratin‐filled acrosyringeal structures …”
