Eccrine poroma is a benign adnexal tumor arising from cells of the outer layer of the acrosyringium and upper dermal eccrine duct. It generally appears as a solitary, slow growing, sessile, pink-to-red and well-circumscribed papule, plaque or nodule. It is usually located on the palms and soles but it may also develop on other locations. Its clinical appearance can resemble other types of tumors such as hypo-or amelanotic melanoma. Dermoscopy has improved the evaluation of skin tumors. In the case of eccrine poroma, there are some studies that have described its dermoscopic findings. These mainly focus on its vascular structures. We present an 82-year-old patient who developed a 2×3-cm eccrine poroma on his lower back. Dermoscopy demonstrated the presence of a polymorphous vascular pattern displaying mostly linear looped (irregular hairpin-like) and -leaf-flower-like‖ vessels (-cherry-blossom‖ and -chalice-like‖), with some resembling -cactus-like‖ structures. Only a few linear coiled (glomerular) and linear helical (corkscrew) vessels were observed. Some of these vascular structures were surrounded by a whitish-to-pink halo. Moreover, some pink structureless areas were present. We highlight the finding of the -leaf-flower-like‖ vessels, as these are vascular structures that have not been described in other types of skin tumors.

Background: Eccrine poroma (EP) belongs to the group of benign adnexal tumors, but cases of eccrine porocarcinoma (EPC) arising on long-standing and untreated EP suggest a certain risk of malignant transformation. Objectives: To describe the clinical, dermoscopic and histopathologic features associated with different extremes in the spectrum of eccrine poroid neoplasms and to review the according literature. Methods: A retrospective analysis of patient characteristics and morphologic features associated with EP, EPC in situ and invasive porocarcinoma patients who attended two skin lesion clinics in Italy and Turkey between 2010 and 2011. Results: A total of 4 cases including 1 EP, 1 EPC in situ and 2 cases of invasive EPC in 4 patients were analyzed. Recent changes including bleeding, ulceration and sudden enlargement of a pre-existing lesion were associated with malignant transformation. Dermoscopically, polymorphous vessels consisting of coiled, hairpin and linear vessels were seen at all stages of progression. Histopathological findings of EPC in situ were limited to the epidermal component and were evident only at higher magnification. Conclusions: Clinicians and pathologists should carefully evaluate EPs with a recent history of change and erosion in order to avoid overlooking the potential development of EPC.

ResumenEl poroma ecrino (PE) corresponde a una neoplasia anexial benigna originada de la porción intraepidérmica del conducto de las glándulas sudoríparas. Habitualmente se describe como nódulos eritematosos o color piel ubicados en región palmoplantar, pero sus características clínicas son muy variables.Se decriben los hallazgos clínicos y dermatoscópicos de una serie de casos de pacientes con PE.La dermatoscopia puede facilitar significativamente el diagnóstico clínico de la mayoría de los tumores de la piel, sin embargo el PE se caracteriza por un alto grado de variabilidad a la dermatoscopía. Habitualmente se describe un patrón vascular polimorfo con vasos glomerulares, en horquilla, y lineales irregulares con terminaciones semicirculares rodeados de un halo blanco a rosa en forma de caliz o flor de cerezo asociado a áreas rosadas blanquecinas sin estructuras similares a lo observado en melanoma amelanótico, carcinoma espinocelular entre otros tumores benignos y malignos.El PE puede exhibir características clínicas y dermatoscópicas polimórficas que pueden hacer difícil el diagnóstico y su diferenciación de otras neoplasias cutáneas.Palabras claves: Poroma ecrino; dermatoscopía; neoplasia anexial cutánea summaRy Eccrine poroma (PE) is a benign adnexal neoplasm of the intraepidermal portion of the sweat gland duct. PE typically present as erythematous or flesh-colored nodules on the palms and soles, but the clinical features of PE are highly variable.Dermatoscopic and clinical findings of patients with EP are described.Dermoscopy may significantly improves the clinical diagnosis of most skin tumours, but PE is characterised by a high degree of dermoscopic variability.
