Dermabrasion for lichen amyloidosus. Report of a long-term study

Abstract: Seven patients with lichen amyloidosus were treated with dermabrasion during the last seven years. All patients have had follow-up examinations for at least five years (one patient for seven years). The main symptom, itching, disappeared in all cases, and the clinical results were satisfactory, except for mild hyperpigmentary and depigmentary changes in the treated areas.

“…[42] Etretinate and acitretin therapy has been beneficial in some cases,[4344] but the condition seems to relapse after the treatment is stopped. [45] Treatment modalities like cyclophosphamide, cyclosporine[46] and dermabrasion[47] have a limited therapeutic efficacy. The Q-switched Nd-YAG laser (532 nm and 1064 nm) has shown positive response in the reduction of pigmentation in MA.…”

“…Potent topical corticosteroids (under occlusion), topical calcineurin inhibitors, topical dimethyl sulfoxide (DMSO), intralesional corticosteroids, PUVA and UVB phototherapy, systemic retinoids, low-dose cyclophosphamide, cyclosporine have been used for medical management of LA. [6] The physical modalities of treatment such as electrodessication,[1] dermabrasion,[23] pulsed dye laser[4] and frequency- doubled Q-switched Nd: YAG laser[5] where epidermis and part of the papillary dermis along with some of the amyloid was removed permitting re-epithelialization to occur from adnexal structures have been reported to be effective in the treatment of LA.…”

“…[23]Since amyloid K is derived from epidermis, fractional ablative effect of the laser could decrease the production of amyloid by affecting the process of keratinization.Erbium: YAG laser penetrates up to upper dermis where the deposits are found. Hence the deposits can get eliminated transepidermally through the channels created by the laser.…”

“…There are reports which have shown the benefits of using oral retinoids , topical calcineurin inhibitors, topical 50–100% dimethyl sulfoxide , cryosurgery , and narrowband UVB . In addition, physical treatments such as scalpel scraping , dermabrasion , elctrodessication , and lasers including the CO 2 laser , pulsed dye laser , Q‐switched Nd:YAG laser , and fractional ablative Nd:YAG laser have all been reported and have shown some clinical improvement.…”
