The objectives of this study were to assess the nutritional status of rural workers from a municipality in Southeastern Brazil and estimate the association of sociodemographic, labor, lifestyle, and dietary pattern factors with obesity and abdominal obesity of men and women of this rural area. This is a cross-sectional, epidemiological study of 740 farmers (51.5%, n = 381 males; 48.5%, n = 359 females). The sociodemographic, labor, lifestyle and dietary patterns determinants were assessed. Food intake data were obtained by applying three 24-hour recalls and dietary patterns were determined by Principal Component Analysis with Varimax orthogonal rotation. Poisson regression with robust variance stratified by sex was applied. The general prevalence of overweight status was 31.5% (95% CI 28.2–34.8%), 19.7% of obesity (95% CI 16.8–22.6%) and 31.5% of abdominal obesity (95% CI 28.2–34.8%), with higher rates in women (P < 0.001). Men of higher socioeconomic class had a 2.3 times higher prevalence of obesity (95% CI 1.08–4.90). In addition, the shorter travel time to purchase food increased the prevalence of abdominal obesity in males. For women, the older the age group, the greater the general and central obesity. A lower adherence to traditional dietary patterns (approximately PR [prevalence ratio] 1.6 for general obesity and PR 1.3 for abdominal obesity) and a greater number of places to buy food were associated with higher rates of obesity in women. Finally, women farmers with a higher workload had a 20% lower prevalence of central obesity (PR 0.80; 95% CI 0.65–0.97). Such findings demonstrate that obesity must be an issue in the health care of remote and rural populations. There is a need to promote healthier environments that respect traditional food culture through multiple approaches that consider the heterogeneity of rural areas and the differences between sexes.

Introdução: As doenças crônicas e os agravos não transmissíveis constituem um relevante problema de saúde pública no Brasil e no mundo. Objetivos: Descrever as ações desenvolvidas para construção da Vigilância das Doenças e Agravos Não Transmissíveis (DANT) na saúde pública do Espírito Santo, com destaque para os trabalhos publicados e eventos desenvolvidos no presente Estado. Métodos: Trata-se de uma revisão narrativa das iniciativas institucionais e das publicações produzidas, contemplando a cronologia dos eventos e das programações desenvolvidas pela Equipe Técnica da Vigilância em DANT na Gerência Estratégica de Vigilância em Saúde. Resultados: As iniciativas promovidas para construção da Vigilância em DANT, segundo a cronologia de publicações, compreendeu 10 Boletins epidemiológicos, 3 Cadernos Epidemiológicos e 1 Informativo da Vigilância do Câncer, publicados no período de 2001-2017. A respeito das iniciativas dos eventos institucionais, foram identificados 6 Seminários Estaduais de Vigilância em DANT. Conclusão: O conjunto de ações desenvolvido permitiu a disseminação da magnitude das DANT sobre o quadro da morbimortalidade no Espírito Santo, contribuindo para fundamentação teórica e advocacy, inserindo o tema em iniciativas como do Plano Estadual de Saúde (2009) e do Plano Diretor de Regionalização (2011), culminando com a exitosa elaboração do Plano de Ações Estratégicas para Enfrentamento das Doenças Crônicas e Agravos Não Transmissíveis no estado Espírito Santo (2017).

Background: Brazil is one of the world’s largest pesticide consumers, but information on pesticide poisoning among workers is scarce. Objective: To evaluate acute pesticide poisoning among tobacco growers, according to different criteria. Methods: This was a two-step cross-sectional study with 492 pesticide applicators. It used a 25 question pesticide-related symptoms (PRS) questionnaire and medical diagnosis for comparison with toxicological assessment. Associations were evaluated using Poisson regression. Results: 10.6% reported two or more PRS, while 8.1% reported three or more. Furthermore, 12.2% received a medical diagnosis of poisoning. According to toxicologists, possible cases accounted for 14.2% and probable cases for 4.3%. PRS increased during the period of greater exposure. Those exposed to dithiocarbamates, sulfentrazone, pyrethroids, fipronil and iprodione exhibited more PRS. The number of exposure types, multi-chemical exposure, clothes wet with pesticides and spillage on the body/clothes were associated with acute poisonings. All criteria showed sensitivity greater than 79% for probable cases but only greater than 70% for medical diagnosis when compared to possible cases, presenting substantial Kappa agreement. Conclusion: The prevalence of acute pesticide poisoning is much higher than officially recorded. Trained physicians can screen for pesticide poisoning. It is necessary to improve workers’ education to reduce pesticide use and exposure to them.
