Delayed formation of a lower second premolar

Abstract: Delayed odontogenesis of a lower second premolar is presented in a case treated without extraction and focuses on the 7-year follow-up of the delayed tooth bud. The follow-up was initiated when the crown formation was diagnosed and was finished when the tooth erupted completely into the orthodontically provided space, which enabled the orthodontic leveling and alignment of the delayed premolar. The long follow-up indicates that delayed tooth buds may develop completely and normally.

“…As permanent tooth buds, excluding wisdom teeth, will commence calcification from birth through to age 3, this would usually not affect the identification of hypodontia in the general population (Schuurs, 2012). Of note however, delayed formation of the lower second premolars has been demonstrated (da Silva Filho et al, 2004; Bicakci et al, 2012; Seager et al, 2013). When looking at a cleft population, Ranta noted that second premolars often formed later when compared with a noncleft population (Ranta, 1986).…”

“…Some published reports recommend waiting for spontaneous eruption owing to a great amount of variability in the formation and eruption of second premolars in general. 26,27 We decided to carefully expose the lower left second premolar and place gentle orthodontic forces on it to simulate natural tooth eruption based on the scientific evidence that hypothesizes the existence of commonalities in the genetic expression and the end results of the 2 processes. 28 The stepwise mechanics for orthodontic traction was planned with the use of the CBCT images.…”
