Deep Parotid Lobe Abscess Presenting with Dysphagia and Trismus

Abstract: An abscess of the deep parotid lobe is an uncommon complication of acute parotitis. Characterized by warm erythematous facial skin and ipsilateral cheek swelling, parotid abscesses have often been associated with decreased saliva production and immunodeficiency. We offer a case of a large deep parotid lobe abscess presenting similarly to a peritonsillar mass, causing significant odynophagia and difficulty swallowing. Computed tomography scan revealed an infected deep parotid lobe sialocele which was surgically… Show more

“…Trismus following traumatic or repeated inferior dental block local anaesthesia will have a relevant timeline, whereas patients presenting with spontaneous trismus may have more serious underlying disease. Whilst a TMJ disorder may be the most likely scenario, tonsillar malignancy may give rise to unilateral facial pain and restriction in opening (47), and an infective cause should also be considered (48). Urgent referral of patients with trismus should be considered when the history and clinical findings are unusual, and symptoms do not resolve.…”

“…Compared with masticator space infection, acute suppurative parotitis manifests with a sudden unilateral tender erythematous mass. The mass could extend from the cheek to the angle of the jaw, leading to trismus [19]. Acute suppurative parotitis can be caused by a calculus (sialolith) or various bacteria and viruses, most commonly Staphylococcus aureus and anaerobic bacteria.…”

“…For example, sectioning the facial nerve will lead to poor preparation of the alimentary bolus during mastication. In many cases this aspect is neglected by the specialist surgeon already troubled by the facial neurologic deficit [30]. Pharyngeal tumors associate swallowing problems from the very beginning.…”
