David's procedure for pulmonary artery aneurysm

Abstract: Pulmonary artery aneurysm (PAA) is a rare entity with fatal complications. Its silent course contributes to large aneurysms with compression symptoms. We present a 39‐year‐old female idiopathic pulmonary arterial hypertension patient with a giant PAA causing severe pulmonary regurgitation (PR) and symptomatic left main coronary artery compression (LMCA). Since she had a failed LMCA stenting attempt, she underwent surgery. A valve‐sparing David‐like pulmonary trunk reconstruction and coronary artery bypass were… Show more

“…Surgery has been used to treat PAAs [14]. Proximal aneurysms may be reconstructed with aneurysmorrhaphy [15,16]. Lobectomy seems to be the most commonly described approach for patients with PAAs and BD [14].…”

“…Reisenauer et al described aneurysmorrhaphy or aneurysmectomy with graft interposition as a surgical option in their single-center experience of 38 patients [6]. Franciosi et al and Circi et al described a Yacoub and a David's procedure to treat a giant PAA in the presence of a normal pulmonary valve, respectively [9,10]. We opted for a porcine root due to its excellent performance and longterm outcomes [11,12].…”

“…The functionality of the pulmonary valve needs to be evaluated to determine if it is affected. If the valve function is normal and only the annulus is enlarged, a procedure similar to David's ( 16 ) or Devega's ( 15 ) can be performed, preserving as much function of the pulmonary valve as possible; if the valve function is abnormal, a bioprosthetic pulmonary valve is also a good option ( 12 , 18 ) ( Figure 3 ).…”
