Malignant melanoma has an unusual predilection for metastasis to the small bowel, sometimes several years after the original diagnosis. In patients who have had a cystoprostatectomy followed by an ileal conduit, metastatic melanoma to the ileal conduit can present in urine cytology. We present a rare case of metastatic malignant melanoma in neo-bladder urine in a patient who had undergone a cystoprostatectomy for high grade urothelial carcinoma of the bladder and prostatic adenocarcinoma of Gleason grade 3+3 and two excisional procedures for cutaneous malignant melanoma. He presented with persistent hematuria and urinary tract infections unresponsive to treatment. Urine cytology revealed single large atypical cells, with large nuclei, prominent nucleoli, and cytoplasmic melanin pigment. Subsequent surgical resection revealed two areas of metastatic melanoma in the ileum, one of them being in the ileal conduit. The tumor cells were immunoreactive for S-100 protein, Melan-A, and HMB-45 and were negative for CAM5.2 and cytokeratins 7 and 20.

| Der Urologe [B] 6•2001 578 Das maligne Melanom der Haut steht in der Häufigkeit bösartiger Erkrankungen an 6. Stelle bei Männern und 7. Stelle bei Frauen. Im Jahr 2001 werden in den USA voraussichtlich 51.400 Menschen daran erkranken und 7.800 Patienten versterben [5]. Inzidenz und Mortalität nahmen in den vergangenen Jahren stetig zu [7]. Die sekundäre Beteiligung des Urogenitaltrakts ist ein häufiger Befund bei Autopsien, aber selten klinisch auffällig. Gelegentlich verursacht die urogenitale Involvierung eine behandlungsbedürftige Hämaturie. Die Prognose dieser Patienten ist wegen der zu diesem Zeitpunkt meist schon disseminierten Erkrankung als schlecht einzuschätzen [2, 3]. Es wurden Fälle mit Harnstauung oder Hämatospermie beschrieben, die durch die Beteiligung des Harntraktes verursacht wurden [1, 6]. Selten findet man ein primäres malignes Melanom in der Urethra oder in der Harnblase [9]. Patienten mit einer sekundären, symptomatischen Manifestation eines Melanoms im Urogenitaltrakt werden einer palliativen Behandlung zugeführt. Ein häufiges Verfahren ist die Harnleiterschienung zur Behandlung einer Harnstauung [4]. Urinzytologie, Probeexzision und Computertomographie wurden als Methoden zur Diagnosestellung beim primären oder sekundären Melanom des Urogenitaltraktes angewendet [8, 10].
