SummaryThe primary cutaneous lymphomas are a heterogeneous group of T-, Natural Killer-and B-cell neoplasms with a wide range of clinical and pathological presentations, and with very different prognoses compared to systemic lymphomas. Recent studies have shown that the skin microenvironment, which is composed of various immune cell subsets as well as their spatial distribution and T-cell interactions through different chemokines and cytokines, has an important role in the development and pathogenesis of cutaneous lymphomas and has assisted in the development of novel and more effective immunotherapies. The following review will focus on the major subtypes of primary cutaneous lymphomas, including the clinical and histological patterns, molecular hallmarks, and current and future treatment strategies.

EinführungPrimäre kutane T-Zell-Lymphome (CTCL) repräsentieren ein Spektrum von T-Zell-Neoplasien, die ihren Ursprung im Hautorgan haben und sich im Verlauf der Erkrankung in die Lymphkno- AbstractIn the beginning of the 19. century the first case reports of patients with cutaneous lymphomas were dominated by a detailed description of the clinical picture. Based on these clinical descriptions and the morphological dermatohistopathological examination of the ¹2. dimensionª of the skin different subgroups of cutaneous lymphomas could be identified. In the late 70s of the 20. century diagnostic criteria were defined based on the morphological description of the clinical and histological features and the general term ¹cutaneous T-cell lymphomaª (CTCL) was established. Finally, with the help of newly introduced immunohistochemical markers a classification of CTCL was worked out by the EORTC and the WHO at the end of the 20. century. Molecular biology (TCR-Rearrangement) offers new insights into CTCL and increases the diagnostic safety especially of early-stage CTCL cases. In the future it will be possible to further improve the diagnosis, prognosis and therapy of CTCL by new discoveries regarding the ¹genetic morphologyª (molecular genetics, microarray-technology) of this skin disease.

2 cases of cutaneous T cell lymphoma are reported. In both cases immunological studies revealed that the tumor cells from the cutaneous lesions had human Ia-like antigen on the cell surface, which was considered to be alloantigen primarily present on B lymphocytes. Serum examination showed an extremely high titer of two types of antibody against Epstein-Barr (EB) virus in the 1 st case and a relatively high titer of one type of antibody in the 2nd case, suggesting that the patients had EB virus infection. Recently, some hematologists found the significant correlation of T lymphocytosis and the appearance of human Ia-like antigen in infectious mononucleosis. Based on these findings, it is assumed that EB virus has the ability of the immunological transformation of cutaneous neoplastic lymphocytes, and so we should not overlook the susceptibility of phenotypical changes of the original cell by EB virus when the immunological classification of cutaneous malignant lymphoma is done.
