Cutaneous periarteritis nodosa. Hepatitis B surface antigen-containing immunocomplexes and polymorphonuclear-leukocyte lysosomal enzyme release

Abstract: In a patient with cold-induced cutaneous periarteritis nodosa, cryoprecipitation of a circulating hepatitis B surface antigen-containing immunocomplex resulted in phagocytosis by neutrophils and monocytes with prominent vacuolation of the cells and extracellular release of lysosomal enzymes. We believe this immunocomplex attached itself to the cell membrane and induced vacuolation and degranulation in normal neutrophils.

“…Cases of CPN associated with inflammatory bowel disease or hepatitis antigenemia have been described recently (36)(37)(38)(39)(40)(41)(42) and immune complexes have been suggested as having an important role in the pathogenesis of the disease (39)(40)(41)(42). In this study, the serologic studies for viral hepatitis and circulating immune complexes of the examined patients were negative or normal.…”

“…Besides hepatitis B virus (41,42), other infectious agents such as Streptococcus (6,43) and Mycobacterium tuberculosis (29) have also been implicated in the pathogenesis of CPN. None of these agents were found in this study.…”

“…Until today, the etiology of CPN has remained unknown. Some authors suggest an association to hepatitis B, 12,13 virus infections or other infectious agents such as streptococcus 8,14 or mycobacterium tuberculosis 15 . Associations with other inflammatory disorders like polychondritis 16 and ulcerative colitis and Crohn's disease 17,18 have been reported.…”

“…Unlike systemic PAN, where up to 30–50% of cases are associated with chronic active hepatitis B infection, 1,2 cutaneous PAN is distinct in that it is usually not associated with hepatitis B infection 3,4 . Furthermore, while there have been reports of cutaneous PAN associated with the wild‐type virus, 5,6 our case was instead associated with the precore mutant virus.…”
