Current views on anthracycline cardiotoxicity

Abstract: Heart Failure Reviews is an international journal which develops links between basic scientists and clinical investigators, creating a unique, interdisciplinary dialogue focused on heart failure, its pathogenesis and treatment. The journal accordingly publishes papers in both basic and clinical research fields. Topics covered include clinical and surgical approaches to therapy, basic pharmacology, biochemistry, molecular biology, pathology, and electrophysiology. The reviews are comprehensive, expanding the re… Show more

“…Although the impaired performance in CTHF is certainly multifactorial, with contributions from loss of skeletal muscle mass and radiotherapy-induced pulmonary disease among others, our findings highlight the importance of LV diastolic impairment in limiting functional capacity in CTHF. Smaller LV dimensions in CTHF compared with NCTHF have been previously reported,9 as has a higher prevalence of recovered LVEF 4. In contrast, similar or even worse LVEF values in CTHF compared with NCTHF have been previously reported by other groups 7 9 10.…”

“…Usually, this happens in the weeks or months after the start of treatment, because anthracycline cardiotoxicity is mainly dose-dependent and produced by a prolonged oxidative stress [3,9,10,11]. However, not only the mitochondrial redox cycling is involved in this toxic process, some recent data imply Top2B, a DNA topoisomerase IIβ, as an essential driver of this mechanism [12].…”

“…This relatively new pathological condition (cancer therapy-induced CVD) has first been revealed nearly fifty years ago with anthracyclines -they are antibiotics derived from Streptomyces that have been widely used as chemotherapy since the late 60s against leukemia, lymphoma, breast, stomach, uterine, ovarian, bladder and lung cancer [6]. Their dose-dependent cardiotoxicity has also been detected back then with asymptomatic myocardial dysfunction, originally detected in children, ranging between 18 and 57% of the survivors -5% die of heart failure [7,8]. CVD displays also greater incidence in people with SCI than in the general population [9].…”
