Objective To identify common ocular and non-ocular signs and symptoms of patients with chiasmal lesions presenting to ophthalmologists. Methods This is a three year case series of patients who presented to Rizal Medical Center Ophthalmology OPD clinic, diagnosed as cases of chiasmal lesion with a complete neuro-ophthalmogical exam, ancillary test and neuroimaging. Data collected include patient profile, ocular and non ocular symptoms, duration of blurring of vision (BOV), best corrected visual acuity (BCVA), Color Vision, Optic nerve (ON) description, Visual field defects (VFD) and size & location of chiasmal lesion. Results A total of twelve patients were included with 1:1 M:F ratio and a median age of 39 years. Blurring of Vision was the presenting symptom in 10 cases, bulging of the eyes and headaches in the remaining cases. Associated symptoms include: non specific headache in all; diplopia in 3 (1 with and 2 without motility defects); and systemic symptoms related to hormonal imbalance in 2 cases. Best corrected vision ranged from 6/6 to no light perception (NLP). Color vision was affected in all cases. ON palor was seen in 6 (50%) and disc edema in 1 case. VFD include Junctional scotoma (58.33%), bitemporal hemianopia (33.3%) and generalised scotoma (8.3%). Neuroimaging revealed pituitary lesions in 10 cases, 1 craniopharyngioma and ON glioma with chiasmal extension. Conclusion Ocular and non ocular symptoms seen were comparable to other studies except that Junctional scotoma was the most common VFD identified in the included population of our case series. This highlights the importance of VF testing among patients complaining of unilateral visual loss to rule out a chiasmal lesion.

Background: Neoplasms of the brain and spine are relatively uncommon compared to breast, lung and gastrointestinal tumours, which occur at higher rates in the Asian population. Updated guidelines in diagnosis and treatment of neuro-oncologic diseases recommend advanced molecular-based precision-medicine; thus the need for increasingly individualised regimens. It is, therefore, necessary to determine whether there are areas of improvement in the provision of care to these patients, especially in low-to middleincome economies like the Philippines. Methods:In this study, we identified gaps in the delivery of medical care to Filipino patients with tumours of the central nervous system. We performed a scoping review on the available literature on clinical experience with treatment of neuro-oncologic cases from the Philippines and performed qualitative analysis viewed through the lens of the existing healthcare system. Results:The medical practice of neuro-oncology in the Philippines lacks robust local data on epidemiology and treatment outcomes. There are existing legislative frameworks to support adequate healthcare delivery and financing to brain tumour patients. However, inequities in the geographic distribution of infrastructure, manpower and medications are roadblocks for accessibility to neuro-oncologic services like specialised molecular markers, neurosurgical procedures, sustained chemotherapy and radiation therapy centres. Conclusion:There are significant treatment gaps in the care of neuro-oncologic patients in the Philippines that need to be addressed. Early detection and initiation of prognosischanging therapeutics through reduction of out-of-pocket expenses, access to readily available diagnostic tools and sustainability of management regimens are the main areas that necessitate strengthened partnership between the public and private sectors of Philippine society.

Introduction: Glioblastoma is the deadliest malignant brain tumors in adults. The main challenges in treating glioblastoma are its resistance to the chemo-radiotherapy, poor outcome and low survival rate. The World Health Organization (WHO) 2016 classification identifies two types of glioblastoma by its mutational status of isocitrate dehydrogenase (IDH); since our national insurance experiences budget-limitation, we could not freely apply it in our institutions. We aims to find the prevalence and outcome of glioblastoma-not otherwise specified (NOS) based on its clinical manifestations and histopathology findings. Methods: We performed retrograde-analysis based on clinical and histology findings on 48 glioblastoma-NOS patients from 2012-2017. We analyzed its characteristic, primary complains, lesions location, macroscopic findings, therapy and the final outcomes. Results: Glioblastoma-NOS is the most common type of gliomas occurs in adults ages 49.29±12.13 years (range 17-72 years). The tumor predominantly involves the frontal lobe (25%) with chronic progressive headache as the chief complaint (90%); 93.8% of the patients underwent tumor removal and received chemo-radiotherapy after surgery based on the histopathology findings. The median survival is 18 months and the prevalence of glioblastoma-NOS in our tertiary referral hospital is 4.72%. Conclusion: Hopefully, our study will improve the understanding of the regional differences in glioblastoma-NOS prevalence and pave the way for identifying the regional risk factors that would allow us to improve the protocols on glioblastoma detection, prevention and management. Further studies, incorporating molecular techniques into a patient’s tumor analysis for IDH1 mutant or wild type are required for the promise of personalized medicine.
