Current treatment modalities of orthodontically induced white spot lesions and their outcome – a literature review

Abstract: Introduction. White spot lesion is a demineralization of the enamel that appears as a white spot on the surfaces of the tooth. The cause of this spot is determined by the activity of the bacterial plaque and it represents the initial stage of a carious lesion. This lesion is a common side effect for patients with fixed orthodontic appliances mainly because of the brackets’ position that favors the accumulation of plaque that ultimately leads to the formation of the white spot.Methods. “…Therefore, the emphasis in the prevention of caries during orthodontic treatment should be on management strategies which focus on the remineralization of the lesions, e.g., the use of topical fluoride [ 13 , 20 , 21 ] and amorphous calcium phosphate [ 35 ]. Moreover, oral hygiene instructions and regular professional cleanings with motivation of patients have been recommended to inhibit demineralization [ 32 ].…”

“…So, it´s all the more important to modify the caries risk factors und emphasis on management strategies which focus on the remineralisation of the lesions. Such strategies are use of topical fluoride [30][31][32] , amorphous calcium phosphate 33 or even a combination of both agents 34 . In addition, masking the lesions would improve the esthetic appearance of the teeth including bleaching, micro-abrasion or micro-invasive techniques such as resin infiltration 35 with a camouflage effect.…”

“…The small relative share of respondents that the body of the braces is a risk factor for the development of these lesions indicates that colleagues are poorly informed that the body of the braces seriously impairs oral hygiene and limits the self-cleaning from the saliva (6). The zones around the fixed braces are preelection areas for the accumulation of a larger amount of dental plaque.…”
