Adjuvant therapies contribute to the successful treatment of cancer. Our previous reports have shown that combining cryoablation with cytotoxic agents enhances cell death. Tumor necrosis factor-related apoptosis-inducing ligand (TRAIL) is a cytotoxic agent that preferentially induces apoptosis in a variety of human cancer cells. Human prostate cancer cells (PC-3) are resistant to many cytodestructive agents, including cryoablation and TRAIL. Here, we evaluated the effects of TRAIL combined with cryoablation on PC-3 and normal prostate (RWPE-1) cell death. Exposure of PC-3 cells to freezing (À101C) or TRAIL (500ng/ml) results in minimal cell death, whereas a complete loss of viability is observed with the simultaneous combination. The synergistic effect was found to be due to a marked increase in apoptosis. Western blot analysis revealed a significant level of caspase-8 and -3 cleavage between 12 and 24 h post-exposure. Caspase activation assays provided similar results and also indicated a role for caspase-9. Inhibitors to caspase-8 and -9 along with a pan-caspase inhibitor were incorporated to determine which pathway was necessary for the combined efficacy. Inhibition of caspase-8 significantly blocked the combination-induced cell death compared to cells that did not receive the inhibitor (63% compared to 10% viable). The addition of the caspase-9 inhibitor resulted in only a minimal protection. Importantly, the combination was not effective when applied to normal prostate cells. The results describe a novel therapeutic model for the treatment of prostate cancer and provide support for future in vivo studies.

ВведениеКриохирургия (греч. kryos (холод) + хирургия) -хи-рургические методы лечения холодом, применяемые в различных областях медицины (хирургия, нейрохирур-гия, онкология, офтальмология, дерматология и др.) [8].В древности медики были хорошо осведомлены о способности холода снимать боль и воспаление, снижать температуру тела, повышать сопротивляемость организма неблагоприятным воздействиям [9]. Существуют доказа-тельства того, что холод применялся в Древней Греции еще за 3500 лет до новой эры при лечении инфицирован-ных ран. Древние египтяне, а позже и Гиппократ, и Гален, и Цельс, и Авиценна знали о болеутоляющих и противо-воспалительных свойствах холода, который использовали для лечения инфицированных повреждений, переломов черепа и различных ран, полученных в бою [9,56,92].Широко использовал охлаждение для лечения ран Н.И. Пирогов. «Холод, безусловно, назначается там, где к опухшей горячей и раздраженной ране присоединяется паренхиматозное (капиллярное) кровотечение», -писал выдающийся хирург [9].Барон de Larrey, французский армейский хирург в пе-риод войны 1812 г., заметил, что если раненые солдаты долго лежали в снегу, то можно было безболезненно про-изводить ампутацию поврежденных конечностей [9]. Спустя столетие, в годы Великой Отечественной войны, этот метод был возрожден выдающимся отечественным хирургом С.С. Юдиным [9].На Руси, как известно, давным-давно было принято после горячей бани окунаться в ледяную купель или рас-тираться снегом.В Европе к этой традиции долгое время относились как к чудачеству, и лишь вышедшее в 1886 г. сочинение баварского священника Себастьяна Кнейпа «Мое водоле-чение» в корне изменило прежнее к нему отношение. Толчком к исследованию целительного фактора холода послужила история, произошедшая с самим автором этой работы. Кнейп описывал, как, будучи больным воспале-нием легких, он, измученный сильным жаром, прыгнул в ледяную воду Дуная, после чего очень быстро пошел на поправку. О важности этого труда можно судить уже по тому, что ни одно сочинение из области гигиены и попу-лярной медицины не переиздавалось столь часто, как книга пастора Кнейпа. Хождение босиком по мокрой тра-ве, влажным камням, холодной воде, а также по свежему снегу названо в ней «самым естественным и простым средством закаливания», а еще способом лечения, осо-бенно для «людей с холодными ногами, горловыми ката-рами, приливами крови к голове и вследствие этого стра-дающих головными болями». Методики Кнейпа стали первым сводом правил холодовой терапии, не потеряв-ших актуальности и сегодня.
