Approximately one-third of patients with coronavirus disease 2019 (COVID-19) present with coagulation disorders and hematological changes. However, the clinical manifestations of COVID-19 and prognoses of people living with human immunodeficiency virus (HIV) remain controversial. This study reports the case of a 27-year-old HIV-infected man who regularly used antiretroviral medications, had no other comorbidities and was admitted for acute respiratory distress syndrome caused by COVID-19. Complementary examinations during hospitalization revealed a diagnosis of pulmonary thromboembolism in association with an intracavitary thrombus.

Purpose of Review Latin America and the Caribbean (LAC) has been hit hard by COVID-19 due to political instability, flawed health systems, and structural inequalities. The repercussion of the pandemic on vulnerable populations, like people living with HIV (PLWH), is complex. This review aims to explore the interactions between the HIV and COVID-19 pandemics in this region. Recent Findings Data regarding the interactions of HIV and COVID-19 in LAC is scarce. Only case reports or small case series have been published regarding the clinical course of COVID-19 in PLWH and regarding the clinical course of COVID-19 in PLWH, which appears to be similar to the general population. The pandemic has disrupted prevention and treatment of PLWH. However, there have been country efforts to counteract those effects. There are some lessons from the HIV response which have been effectively applied in the region to address COVID-19. Summary COVID-19 has had an unprecedented impact on the cascade-of-care among PLWH in LAC. There is a need for longitudinal studies that assess clinic implication of these pandemic interactions in LAC.

“…Kowalska et al, (2020) Além disso, desde o começo da pandemia, os pesquisadores relataram resultados de um pior prognóstico para pacientes com doenças pré-existentes, descrevendo que a imunossupressão do HIV poderia aumentar a suscetibilidade a COVID-19, isso porque linfócitos T CD4+, presentes em menor quantidade em PVHA, estão envolvidos de maneira fundamental na resposta imunológica ao coronavírus, não sendo possível a diferenciação das células B e produção de anticorpos suficientes, ou seja, PVHA, sem tratamento adequado não possuem resposta imunológica efetiva contra o SARS-CoV-2 (Chen et al, 2022). Indo ao encontro desse estudo, alguns pesquisadores relataram que PVHA apresentam um risco menor de complicações decorrentes da COVID-19 se em tratamento regular, uma vez que o uso de ARV influência para um menor risco de inflamação, bem como, aumento da efetividade do sistema imunológico, no combate ao vírus (Cipolat & Sprinz, 2020;Karmen-Tuohy et al, 2020). Ainda não se pode afirmar que PVHA possuem maior suscetibilidade à COVID-19 na forma grave, nem que a terapêutica usada por PVHA pode exercer um efeito protetor, porque deve-se considerar outros fatores epidemiológicos e os relacionados a doença de fato, considerando a fase da doença, se em tratamento regular, e os níveis de CD4+ e CV (Kanwugu & Adadi, 2021).…”

O presente estudo teve como propósito delinear o perfil clínico-epidemiológico de pessoas vivendo com HIV/AIDS (PVHA) internadas com suspeita de COVID-19. Tratou-se de um estudo longitudinal, observacional e de caráter retrospectivo, através da análise de prontuários clínicos de PVHA, atendidos no Hospital Universitário Maria Aparecida Pedrossian, localizado em Campo Grande, MS, entre março de 2020 a dezembro de 2021. Participaram da pesquisa 92 casos de PVHA que permaneceram internadas por no mínimo 24h, com suspeita de COVID-19, tendo como sinal prevalente na admissão hospitalar a dispneia (70%). A amostra (n=92) foi dividida em dois grupos: Grupo 1, composto por PVHA que testaram positivo para COVID-19 (n=23) e Grupo 2, composto por PVHA que testaram negativo para COVID-19 (n=69). Concluiu-se que houve predomínio de homens, com idade entre 44 a 53 anos, em ambos os grupos. Os pacientes internados sem COVID-19 tinham uma condição de imunossupressão pior (níveis de CD4 + < 200, carga viral detectável e imunossupressão severa) e apresentavam maior prevalência de doenças oportunistas. De forma semelhante, ambos os grupos estudados desenvolveram quadros de SRAG, com saturação menor que 95%, necessidade de UTI e necessidade de VMI. Quanto ao desfecho final, não houve diferença entre os grupos.
