Background/Aim:Emergency surgeries have increased in Saudi Arabia. This study examines these surgeries and associated complications.Patients and Methods:This was a prospective multicenter cohort study of patients undergoing emergency intraperitoneal surgery from the eight health sectors of Saudi Arabia. Patients' data were collected over 14 days.Results:In total, 283 patients were included (163 men [54.06%]). The majority of cases were open surgery (204 vs. 79). The 24 h and 30-day mortality rates for the cohort were 0.7 and 2.47%, respectively. Twenty-nine patients (10.24%) required re-intervention, while 19 (8.12%) needed critical care admission. The median length of hospital stay was 3 days. Multivariate analysis showed American Society of Anesthesiologist (ASA) classification score (P = 0.0003), diagnosis (P < 0.0001), stoma formation (P = 0.0123), and anastomotic leak (P = 0.0015) to correlate significantly with 30-day mortality.Conclusion:American Society of Anesthesiologist score, diagnosis, stoma formation and anastomotic leak are associated with 30-day mortality after emergency surgery in Saudi Arabia.

Цель исследования-изучить результаты применения методов ускоренной реабилитации у пациентов с острым аппендицитом. Материал и методы. Проведено проспективное рандомизированное многоцентровое исследование среди 86 (38-основная группа и 48-контрольная) пациентов с лапароскопической аппендэктомией и лигатурным способом обработки культи червеобразного отростка под эндотрахеальным наркозом. Протокол включал: информирование, отказ от премедикации, инфузию глюкозы перед операцией, антибиотикопрофилактику, удаление брыжейки, ограничение дренажей, внутрибрюшную пролонгированную анестезию, мини-пневмоперитонеум, ограничение ирригации, монополярное электрохирургическое воздействие минимальной мощности, антиэметики, ранние активизацию и прием жидкости и жидкой пищи (2 и 6 ч после операции). Боль оценивали по визуально-аналоговой шкале, перистальтику-аускультативно каждые 2 ч после операции. У 11 (29%) пациентов основной и у 15 (31%) пациентов контрольной группы уровень кортизола плазмы исследовали до, через 6 и 12-24 ч после операции. Критерии выписки: отсутствие лейкоцитоза, лихорадки и болевого синдрома, требующего назначения анестетиков, отсутствие признаков осложнений и согласие пациента. Результаты. Пациенты не различались по длительности заболевания, полу, возрасту и характеру основной и сопутствующей патологии. Продолжительность операции в условиях мини-пневмоперитонеума и стандартного давления не различалась: 69,2±3,98 и 70,9±3,89 мин (р=0,762). Болевой синдром и потребность в анальгетиках были достоверно меньше в основной группе во всех временны`х интервалах. У 16 (42%) пациентов основной и 2 (4,1%) пациентов контрольной групп болевой синдром в 1-е сутки после операции отсутствовал (0-1 балл). В основной группе френикус-синдром был у 36,8%, в контрольной-у 60,4% пациентов (p=0,05). Различий в группах по диспепсии и времени активизации перистальтики не выявлено. Продолжительность госпитализации в основной группе составила 1,45 сут, в контрольной-3,15 сут (р=0,002). В группе с применением методов ускоренной реабилитации 18 (47%) пациентов были выписаны в 1-е сутки после операции, в контрольной-4 (8,3%) (p<0,001). Повторных госпитализаций не было. Уровни кортизола после операции у пациентов основной и контрольной групп не различались. Также не отмечено различий у пациентов с осложненным и неосложненным послеоперационным периодом. В основной группе у 1 пациента развился послеоперационный парез кишечника (2-я степень тяжести по классификации Clavien-Dindo). В контрольной группе у 7 пациентов сформировался послеоперационный инфильтрат, у 1-парез кишечника (2-я степень тяжести по классификации Clavien-Dindo). У 3 из 7 пациентов с послеоперационным инфильтратом был установлен дренаж после операции; 6 больных получали антибиотикотерапию. Всем пациентам с осложнениями проведено успешное консервативное лечение. Заключение. Методы ускоренной реабилитации у больных при остром аппендиците способствуют уменьшению выраженности болевого синдрома, снижению количества осложнений и сокращению сроков госпитализации. IntroductionThe perioperative management of biliary disease (BD) is variable across institutions with suboptimal outcomes for patients and health care systems. This results in inefficient utilization of limited resources. The aim of the current study was to identify modifiable factors impacting patients' time to theater, intraoperative time, and time to discharge as the constituents of length of stay to guide creation of a perioperative management protocol to address this variability.MethodsData were prospectively captured at Christchurch Hospital for all adult patients presenting for cholecystectomy between May 2015 and May 2022. Pre, post, and intraoperative factors were assessed for their impact on time to theater, operative time, and postoperative hours to discharge.ResultsFour thousand five hundred seventy‐seven patients underwent cholecystectomy during the study period, of which 2807 (61%) were acute presentations and made up the cohort for analysis. Time to theater was significantly impacted by preoperative imaging type, while operative grade and the procedure type had the most clinically significant impact on operative time. Postoperatively time to discharge was significantly impacted by drain placement.ConclusionsStandardizing management of BD would likely result in significant savings for the health care system and improved outcomes for patients. The data seen here evidence the importance of appropriate imaging selection, intraoperative difficulty operative grade identification, and low suction drain selection. These data have been incorporated in a perioperative management protocol as standardization of care across the patient workflow in BD is a sensible approach for ensuring optimal use of scarce resources.
