Topical corticosteroid formulations have been evaluated by visual grading protocols for many years. Toward a more objective methodology, several instrumental methods have been evaluated for applicability in quantifying the vasoconstriction side-effect that follows corticosteroid application to the skin. Although the chromameter has been adopted by regulatory bodies throughout the world as the current standard for topical bioequivalence determinations, there is considerable criticism of this instrument from several quarters. A preliminary comparison reported here indicates that digital image analysis provides statistically significant results that are similar to those obtained by visual assessment techniques, and shows considerably greater precision than that obtained by the chromameter. Continued evaluation of objective assessment techniques, such as digital imaging, and continued modernisation of regulatory bioequivalence requirements will assist in protecting patients and optimising clinical results.

Corticoides Tópicos: es la terapia más prescrita y utilizada para el tratamiento de la psoriasis existen diferentes presentaciones disponibles como son las cremas, los emolientes, las lociones, las soluciones o los aerosoles antiinflamatorios y antiproliferativos, siendo el eje de acción habitual en las lesiones psoriásicas y según Stoughton-Cornell, la clasificación se basa en la potencia y en la capacidad vasoconstrictora de cada uno de ellos, que a la vez está relacionada con su potencia antinflamatoria 36 . RESUMENLa psoriasis es una enfermedad cutánea, no contagiosa, de carácter inflamatorio, ge neralmente de curso crónico y recidivante. Afecta al 3% de la población, siendo el origen idiopático, si bien parece estar relaciona da con el sistema inmunitario y con factores ambientales en personas con una susceptibilidad genética. Las lesiones son usualmente distribuidas simétricamente y están característicamente localizadas a nivel general en las orejas, los codos, las rodillas, el ombligo, la hendidura glútea, los genitales y el cuero cabelludo y de manera especial en los pies y en las uñas. Esta afección ocasiona perturbación ocupacional, deterioro psicológico y físico, y una potencial estigmatización social.La carencia de consenso en relación a la etiología multifactorial que presenta esta afección y la prevalencia de esta enfermedad a nivel del pie, nos lleva a mejorar los conocimientos existentes en lo que se refiere a su clasificación, diagnóstico y tratamiento en la búsqueda de actualizar los conceptos relacionados con esta enfermedad no transmisible. Background: Corticosteroids are extremely useful in the treatment of inflammatory skin disorders. Topical steroid applications are the most effective treatment for all types of psoriasis. Aims:To compare the efficacy of once daily versus alternate day application of topical steroid clobetasol propionate 0.05% (Tenovate cream®) in patients who have mild to moderate plaque psoriasis. Settings and Design:This study was conducted on 89 patients of plaque psoriasis, who attended the skin OPD in our hospital. Methods and Material:The patients who had a mild to moderate severity of plaque psoriasis were selected. Those patients who required systemic corticosteroids, those who were already undergoing any psoriasis treatment, those who had any other debilitating illness and pregnant and lactating women were excluded from the study. Eighty nine patients were enrolled for the study after taking written informed consent from them and they were randomly allocated into two groups. Two patients dropped out, one in each group and 1 of group 2 was prescribed systemic corticosteroids.Group 1-once daily application (n= 44) Group 2-alternate day application (n= 42) An objective scoring was done on the basis of the PSI (Psoriasis Severity Index) score, which was graded from 0-4. Follow ups were done in the 2nd, 4th and 6th weeks.Statistical Analysis: It was done by the Student's 't' test and ANOVA.
