Correction of Microstomia Reconstruction With the Use of Acellular Dermal Matrix for Buccal Reconstruction

Abstract: Correction of microstomia is challenging with a high rate of recurrence. We report the successful treatment of microstomia using acellular dermal matrix (ADM) as an adjunct for intraoral lining with >1 year of follow-up. A 9-year-old international patient with severe immunodeficiency presented with severe microstomia because of recurrent oral infections. She had undergone 3 previous failed attempts to re-establish an adequate oral opening and was dependent on enteral nutrition via gastrostomy tu… Show more

“…There are studies in the literature with Dermal Matrix showing good results, reducing scar retraction in human adults. 10,11,[27][28][29][30][31][32] Schmitt suggests that the Mucograft ™ (Geistlich Pharma AG, Wolhusen, Switzerland) ® membrane is a promising alternative for the regeneration of keratinized mucosa and is a sufficient substitute for the free gingival graft. 10,11 The dermal matrix was designed for soft tissue regeneration as an alternative to autogenous free gingival graft and connective tissue graft.…”

“…Surgical techniques that cover the denuded palatine bone by replacing the mucosa with autogenous grafts from the face, palate, and periosteum or by oral mucosal membranes obtained through tissue engineering have been tested to promote better healing and have beneficial effects. [27][28][29][30][31][32] Theoretical disadvantages of DM in comparison to autologous grafts are risk of infection, transmission of disease, cost factor and lastly religious diversity.…”

“…The applications of ADMs in reconstructive surgery have become quite common over the past 20 years. ADMs are currently far more used in reconstructive surgery than in burns, as a soft tissue implant for augmentation and reconstruction of different body areas such as: tympanic membrane (23), cleft palate (24), dura mater (25), nose (26), lips (27), chest (28), abdominal wall (29), upper and lower extremities (30,31), pelvis etc. (32,33,34).…”
