Purpose In skeletally immature patients, treatment of malalignment about the knee is possible by performing temporary hemi-epiphyseodesis. Following the well-established procedure of physeal stapling, the 8-plate was introduced as a new device. The purpose of this study was to compare physeal stapling with 8-plate hemi-epiphyseodesis. We focused on evaluating deformity correction, complication rate and duration of the procedures. Methods We retrospectively analysed 35 patients (61 extremities, age 2.9-16.0 years) who were treated by temporary hemi-epiphyseodesis about the knee for correction of genu varum or genu valgum by using Blount staples (32 extremities) or the 8-plate (29 extremities). Plain radiographs were analysed at the time of operation and at hardware removal that included measurement of mechanical axis deviation, mechanical lateral distal femoral angle and mechanical medial proximal tibial angle. Time until hardware removal, operation time and complications were recorded. Results A statistically significant improvement of all radiographic measurements could be achieved with comparable results in both groups. Complications were similar in both groups with no relevant differences in amount and severity. In the 8-plate group, however, the surgical time was significantly shorter by an average of ten minutes for implantation and 12 minutes for explantation.Conclusions Both Blount stapling and the 8-plate technique are methods for correction of genu varum and valgum deformity in skeletally immature patients; however, a shorter operating time for implantation and explantation was noted for the 8-plate technique.

Різниця довжини нижніх кінцівок (РДНК) є досить поширеною патологією в дітей та дорослих, вона має поліетіологічний характер. За даними різних авторів, РДНК зустрічається в 40–70 % населення в популяції. Клінічне значення РДНК у дітей визначається комплексними біомеханічними та анатомо-фізіологічними порушеннями опорно-рухового апарату. Етіологічний чинник та ступінь вираженості РДНК (незначний, помірний, значний) визначає тактику подальшого лікування. Для лікування РДНК у дітей запропоновано багато різноманітних хірургічних методів. Корекція РДНК проводиться шляхом вкорочення більш довгої кінцівки або подовження сегментів вкороченої кінцівки. Методи корекції РДНК, механізм дії яких базується на керуванні функцією наросткової зони (НЗ) довгих кісток, є перспективними, упродовж останніх 80 років вони були об’єктом пильних досліджень та експериментальних пошуків. Сучасна тенденція лікування ортопедичної патології в дітей орієнтована на використання малоінвазивних методик хірургічного лікування, саме тому концепція керованого росту, що передбачає використання різних методів впливу на функцію НЗ (епіфізеодез, степлювання, PETS-техніка, тимчасове блокування пластинами), набуває широкої популярності. Саме лікування помірної РДНК (3–6 см) у дітей може ефективно проводитися з використанням методів керованого росту. Використання тимчасового двобічного блокування НЗ пластинами з гвинтами є перспективним напрямком, адже дозволяє проводити оборотне гальмування росту здорової (довшої) кінцівки, чим досягається лікувальний ефект, при цьому значно розширюються показання до використання цього методу. У висновках можна зазначити, що оптимізація та вдосконалення методів лікування РДНК у дітей за рахунок використання малоінвазивних методик, що впливають на функцію НЗ, є актуальною та недостатньо вивченою проблемою, яка потребує подальших досліджень.

Tratamento cirúrgico das dismetrias e deformidades angulares de membros inferiores mediante epifisiodese com parafusos transfisários (técnica de Metaizeau) ResumenEvaluamos los niños con dismetrías y deformidades angulares operados mediante la Técnica de Metaizeau en el período 2010-2014, mediante un estudio retrospectivo de 11 pacientes, analizados en 2 Grupos. Grupo 1 (Dismetrías), formado por 5 pacientes, de los cuales 3 fueron operados solamente mediante esta técnica, mientras que 2 requirieron adicionalmente procedimientos de alargamiento femoral, valoramos dismetría inicial, dismetría por predicción para el último control y para el final de la madurez, y dismetría final. Grupo 2 (Deformidades Angulares), formado por 6 pacientes, de los cuales 1 presentaba genu varo, 4 genu valgo y 1 valgo de tobillo, valoramos el ángulo femorotibial anatómi-co, ángulo femoral distal lateral anatómico, ángulo tibial proximal medial, y ángulo tibioastragalino. En ambos Grupos se valoraron las complicaciones. El seguimiento promedio fue de 2 años y 5 meses. En el Grupo 1 obtuvimos dismetría promedio inicial 7,64 cm., dismetría promedio por predicción para el último control 8,54 cm., dismetría promedio final 7,62 cm., corrección promedio lograda 0,91 cm. En el Grupo 2 el promedio de corrección del ángulo femorotibial anatómico 16,85˚, promedio de corrección del ángulo femoral distal lateral anatómico 16,85˚, promedio de corrección del ángulo tibial proximal medial 7,62˚. No detectamos complicaciones en ningún paciente. En 8 (72,72%) pacientes la Técnica fue efectiva, mientras que en 3 (27,27%) los resultados fueron malos por errores técnicos o error en la predicción. La Técnica de Metaizeau, respetando los detalles quirúrgicos y realizando una adecuada predicción, es una buena opción para el tratamiento de dismetrías y deformidades angulares.
