IntroductionAIDS enters its third decade as an epidemic of great transcendence, affecting the quality of life of the population, principally young adults, and having a great impact on the economy and societal and family structures. It is estimated that at the beginning of the present century, worldwide, there are 40 million people living with HIV, with 4 million new infections per year, and 2.8 million deaths worldwide 1 .Substantial changes in the levels of morbidity-mortality have been recorded, due to the introduction of highly active antiretroviral therapy (HAART), starting in 1996 2,3 , with a significant increase in life expectancy in places where HAART is available, and furthermore with marked socioeconomic 4 and gender differentials 5 .The majority of people infected with HIV or AIDS in developing countries have not benefited from, as they do not yet have access to, therapies that are presently available in developed countries, presenting a profound inequality in relation to access to essential medications 6 .This article reviews the principal epidemiological findings of the Brazilian epidemic and summarizes the results of national research through articles published in journals indexed in MEDLINE and/or SciELO databases up to 2006, as well as via complementary analyses of databases of AIDS cases reported to the Brazilian Ministry of Health (Reportable Diseases Da-REVISÃO REVIEW

Chagas' disease is a major public health problem in Latin America. About 16 million persons are affected and 90 million others are exposed to the risk of being infected by the parasite. The knowledge of epidemiological aspects of the disease allowed to delineate the strategies for the control of the disease related with the vectorial transmission. However, these strategies have had no priority in all endemic countries. Rural-urban migration in most endemic areas carried infected individuals to urban centers increasing the problem of Chagas' disease by blood transfusion. In Brazil the control program has reached good results in the last years and in several states the vectorial transmission was controlled. More recently, hemotherapic practices are performed using screening procedures but this practice must be improved in order to eliminate the possibility of Chagas' disease transmission by another ways (congenital, accidental, oral, etc.). An adequate health care to the infected persons must be improved in order to diminish the social costs of the severe cardiopathy which has been responsible for the adults premature deaths.

Path of infectious diseases in Brazil in the last 50 years: an ongoing challenge

In this article, we comment on the main features of infectious diseases in Brazil in the last 50 years, highlighting how much of this path Revista de Saúde Pública could portray. From 1967 to 2016, 1,335 articles focusing on infectious diseases were published in Revista de Saúde Pública. Although the proportion of articles on the topic have decreased from about 50.0% to 15.0%, its notability remained and reflected the growing complexity of the research required for its control. It is noteworthy that studies design and analysis strategies progressively became more sophisticated, following the great development of epidemiology in Brazil in the recent decades. Thus, the journal has followed the success of public health interventions that permitted to control or eliminate numerous infectious diseases – which were responsible, in the past, for high rates of morbidity and mortality –, and also followed the reemergence of diseases already controlled and the emergence of until then unknown diseases, with a strong impact on the Brazilian population, establishing a little predictable and very challenging path.

To describe the epidemiologic profile and prevalence of cardiopathy in 163 Trypanosoma cruzi serum positive blood donor candidates, a descriptive study was carried out between August, 1996 and November, 1997 at the Londrina State University Chagas Disease Outpatient Clinic. The profile found was: young, average age 42.95 +/- 8.62 years; male (65%); Caucasian (84%); low level of schooling; low family income; agricultural worker (26%); born in the state of Paraná (67%); from rural areas (85%); migrated to the city (85%); and the vector as the main mechanism of transmission. During the clinical characterization a chronic cardiac form was found in 38% of the patients and classified as cardiac suggestive form in 21% and little suggestive of Chagas disease in 17%. No significant difference was found among age group distribution, sex and the presence of cardiac symptoms in patients with or without cardiopathy. This study emphasizes the importance of expanding medical services to areas with a greater prevalence of infected individuals, in a hierarchical manner and aiming at decentralization.
