Contemporary Management of Cardiogenic Shock: A Scientific Statement From the American Heart Association

Abstract: Cardiogenic shock is a high-acuity, potentially complex, and hemodynamically diverse state of end-organ hypoperfusion that is frequently associated with multisystem organ failure. Despite improving survival in recent years, patient morbidity and mortality remain high, and there are few evidence-based therapeutic interventions known to clearly improve patient outcomes. This scientific statement on cardiogenic shock summarizes the epidemiology, pathophysiology, causes, and outcomes of cardiogenic shock; reviews … Show more

“…Invasive haemodynamic monitoring is not routinely advocated, 1 and we believe that our cohort reflects current clinical practice. The observation that miR-423-5p level above median was associated with a lower cardiac index is limited by the small proportion of patients (21%) with pulmonary artery catheters.…”

“…Future studies will help parse out specific patient-device-timing combinations to optimize outcomes. There has been a recent scientific statement on cardiogenic shock from the American Heart Association reviewing contemporary medical, surgical, mechanical circulatory support treatment approaches, and future research initiatives (38).…”
