Crosstalk between T cells, dendritic cells, and macrophages in temporal leukocyte clusters within barrier tissues provides a new concept for T cell activation in the skin. Activated T cells from these leukocyte clusters play critical roles in the efferent phase of allergic contact hypersensitivity (CHS). However, the cytokines driving maintenance and survival of pathogenic T cells during and following CHS remain mostly unknown. Upon epicutaneous allergen challenge, we here report that macrophages produce IL-27 which then induces IL-15 production from epidermal keratinocytes and dermal myeloid cells within leukocyte clusters. In agreement with the known role of IL-15 as a T cell survival factor and growth cytokine, this signaling axis enhances BCL2 and survival of skin T cells. Genetic depletion or pharmacological blockade of IL-27 in CHS mice leads to abrogated epidermal IL-15 production resulting in a decrease in BCL2 expression in T cells and a decline in dermal CD8+ T cells and T cell cluster numbers. These findings suggest that the IL-27 pathway is an important cytokine for regulating cutaneous T cell immunity.

Introduction: Nuss procedure is the main reason for metal implants use by pediatric and thoracic surgeons. There is an ongoing debate on how to avoid allergic complications. Herein we describe our eight-year experience with systematic preoperative metal patch testing and our selective titanium bar use in Nuss procedure. Materials and Methods: Single center retrospective observational cohort study of patients who underwent the Nuss procedure from 2013 to 2020. Preoperative metal patch testing was done in all cases. Criteria for titanium bar utilization were: a positive test for a major component of the stainless-steel bar; or a positive metal patch test and a positive history of atopy, food or metal allergy, or previous allergic reaction to an implant or device. Results: 56 patients were included. Most were male (91.1%) with a median age of 15.0 [13.0-22.0] years old. 19.6% had a positive preoperative metal patch test and 54.5% of these had no personal history of atopy. Stainless-steel bars were used in 27.3% of those patients and titanium bars were used in 72.7%. One patient had a documented minor allergy reaction. None of the 56 patients required early bar removal. Conclusions: Our study suggests that routine preoperative allergy testing and a judicious use of titanium bar is safe and avoids metal allergic complications.

Background: Irritant Contact Dermatitis (ICD) is an eruption of the skin caused by the non-immunologic inflammatory response of the skin to the irritant agents. The government urged to implement health protocols 3M, one of them is hand hygiene by used hand sanitizer to prevent the spread of COVID-19 infection. Repeated use of hand sanitizer can cause skin irritation and inflammatory response such as dryness, cracks, and other symptoms of ICD.Objective: To analyze the relationship between the frequency of use of hand sanitizer and the incidence of ICD.Methods: Observational analytics study cross-sectional design. The research subject were 260 medical faculty students of Diponegoro University who had to agree with informed consent, filled questionnaires and qualified by research criteria. Diagnostic of ICD based on questionnaires and physical examination of ICD. The statistical analysis data was used Chi-square test.Results: The results showed after analyze data use Chi-square test that there was not significantly related frequency use the hand sanitizer to the incidence irritant contact dermatitis during the COVID-19 pandemic with a p-value is 0,078 (p>0,05). The most frequent use of hand sanitizers is 5-10 times a day (56,5%), the most symptoms in ICD are dry, scaly, and peeling skin (30%), and the incidence of ICD due to the use of hand sanitizers are 33,5%.Conclusion: There was no significant relationship between frequency of use of hand sanitizer and irritant contact dermatitis during pandemic the COVID-19.
