IntroductionFacial nerve has the longest and complex course in its bony canal. The anatomical variations make the nerve prone to injury during mastoid surgeries. Having a thorough anatomical knowledge and its variations is must for the surgeons to avoid injury to this vital structure and for the safe surgery. The objective of the study was to describe the anatomical variations of intratemporal portion of facial nerve.Materials and methodsThe study was conducted in the department of otorhinolaryngology in the temporal bone dissection laboratory of a tertiary health care centre. Fifty wet temporal bones were dissected by the same team of surgeons for the duration of one year to study the various anomalies of the intratemporal course of the facial nerve and its relations with the other important middle ear structures.ResultsThe mean length of the labyrinth, tympanic and mastoid segment of the facial nerve was 4.1 mm (±0.6 mm), 9.34 mm (±1.12 mm) and 12.8 mm (±1.8 mm), respectively. The mean distance between the horizontal segment and posterior edge of the oval window was 3.1 mm (±1.03 mm). Dehiscence in the tympanic segment was observed in three temporal bones (6%). Two (4%) specimens had bifurcation of the mastoid segment of the facial nerve. In all dissected temporal bones, the chorda tympani travelled in an ascending path.ConclusionThe most common site for facial nerve anomaly is the tympanic portion. Anomalous conditions that can place the nerve at risk of being injured by the surgeons are highlighted here.

The anatomical position of the facial nerve is a critical factor in determining surgical candidacy in patients with congenital aural atresia (CAA). All patients with CAA must preoperatively be evaluated using a grading score based on information gained from a high resolution CT scan. In patients not suitable for surgical reconstruction, implantation of novel hearing implants is increasingly used for hearing rehabilitation. We, here, describe a bifurcation of the intratemporal part of the facial nerve in a 5-year old boy with CAA undergoing implantation with a bone conductive hearing device.

Tədqiqatın məqsədi braxio-, dolixo- və mezokran tipli kəlləyə malik kişi və qadınlarda üz kanalının labirint və məməyəbənzər seqmentlərinin uzunluğunu dəyərləndirmək olmuşdur.Tədqiqatın materialını Azərbaycan Tibb Universitetinin Tədris Cərrahiyyə Klinikasının Radiologiya şöbəsinin və Şüa diaqnostikası və terapiyası kafedrasının arxivindən götürülmüş 133 pasiyentin kəlləsinin kompüter tomoqramları(KT) təşkil edir. Bütün kəllələr kəllə indeksinə görə braxiokranlara (80,0 və yuxarı), dolixokranlara (74,9 və aşağı) və mezokranlara (75,0-79,9) bölünmüşdür. Üz kanalını üç hissəyə ayrılmışdır: labirint, timpanik və məməyəbənzər (və ya şaquli). Labirint seqmentinin uzunluğunu - üz kanalının girəcəyindən birinci dizciyin ortasına qədər olan məsafəni və məməyəbənzər və ya şaquli seqmentin uzunluğunu - ikinci dizciyin ortasından biz-məməyəbənzər dəliyə qədər olan məsafə ölçülmüşdür.Tədqiqatın nəticələri göstərdi ki, braxiokranlarda, həm sağda, həm də solda kişilərdə və qadınlarda labirint hissəsinin uzunluğunu müqayisə edərkən (kişilərdə sağda - 2,29 ± 0,05, solda - 2,30 ± 0,04; qadınlarda müvafiq olaraq - 2,45±0,04 və 2,42±0,03) əhəmiyyətli fərqlər meydana çıxır (sağda - 0,013, solda - 0,006). Bu göstəricilərin Student-Bonferroni metodu ilə təhlili də yuxarıda deyilənləri təsdiqlədi. Müxtəlif kəllə indeksləri olan hər üç kəllə qrupunu (sağda, 0,013; solda, 0,012) müqayisə edərkən qadınlarda da əhəmiyyətli fərqlər müşahidə edilmişdir. Beləliklə, qadın braxiokranlarda bu göstərici sağda - 2,45±0,04, solda - 2,42±0,03; dolikokranlarda müvafiq olaraq - 2,33±0,04 və 2,34±0,03; mezokranalarda müvafiq olaraq - 2,27±0,03 və 2,29±0,02 . Mann-Whitney metodundan istifadə edərək qrupları müqayisə edərkən, yalnız kişilərdə məməyəbənzər uzunluq sağda braxiokranlar (7,38±0,22) və dolikokranlar (8,08±0,22) arasında əhəmiyyətli fərqlər (0,036) müşahidə edilmişdir. Statistik analizin müxtəlif üsullarından istifadə edərək digər qrupların müqayisəsi etibarlı nəticələr veməmişdir.Yekun. Seqmentlərin uzunluğunun ölçülməsinə dair əldə edilən məlumatlar göstərdi ki, braxiokran qadınlarda labirint seqmentinin üz kanalının birinci dizciyinin ortasına qədər uzunluğu kişilərə nisbətən daha çoxdur. Üz kanalının ikinci dizcikdən biz-məməyəbənzər dəliyə qədər uzunluğu kişilərdə dolixokranlarda sağda kəllələrin digər formalarına nisbətən daha uzundur.
