Background: Laryngeal cancer is relatively uncommon in Ecuador. Usually epithelial in origin, the most frequent histological type is squamous cell carcinoma. The most common mesenchymal tumor is chondrosarcoma. Most laryngeal chondrosarcomas are treated with total laryngectomy, but a conservative function sparing resection is recommended in low-grade limited tumors. Case Report: In a 68-year-old female nonsmoker patient, a small tumor was found in the posterior left aspect of the cricoid cartilage in a computed tomography (CT) performed immediately after an unexpected difficulty to pass the endotracheal tube for a thoracoscopic biopsy of 4 cm tumor of the left lung, in another hospital. The patient underwent, then, an initial tracheostomy, a total thyroidectomy for a goiter and a biopsy of the tumor of the cricoid cartilage whose pathological study was inconclusive. One month later, a low-grade neuroendocrine pulmonary tumor was completed resected. Two years later, a CT scan showed the cricoid lesion with the same characteristics. At endoscopic video laryngoscopy, two subglottic masses that narrowed the airway in approximately 60% of the normal caliber, were observed located at the posterior and left walls. An intraluminal resection was performed through a transcricoid anterior approach. The pathological diagnosis was a low-grade chondrosarcoma. Tracheal decannulation was performed one month later. At an endoscopic video laryngoscopy performed six months post-operatively, the tracheal caliber and mucosa were normal. The patient remained with normal voice and breathing. Conclusion: We report the second case of chondrosarcoma of the larynx in our country, treated by a conservative approach.

SUMMARY Laryngeal chondrosarcomas (LCS) are rare lesions that behave as locally aggressive tumours, producing symptoms such as dysphonia, dyspnoea, dysphagia and hoarseness. Different approaches for the treatment of LCS have been described in the literature. The main purpose of this investigation was to find all cases of LCS published to date and analyse management data and survival outcomes. In December 2020, a systematic review was performed following Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines and the Provider Enrollment, Chain, and Ownership System (PECOS) method including articles published up to this date on different aspects of LCS. The search included the terms larynx, laryngeal, chondrosarcoma. A total of 148 articles were included in this systematic review describing 381 cases of LCS. Dyspnoea was the most reported symptom followed by hoarseness and neck mass sensation. Cricoid cartilage was the most usual location. Survival rate was influenced by both surgical technique (log-rank = 11.7; p = 0.008) and the degree of tumour histologic differentiation (log-rank = 18.3; p = 0.003).

Cirugía de salvamento en un caso de condrosarcoma de rodilla

El siguiente caso, se trata de una paciente de sexo femenino que acude a consulta de traumatología por dolor de rodilla y dificultad para deambular, sin antecedente de trauma. En la radiografía anteroposterior y lateral se evidencian múltiples áreas radiolúcidas y escleróticas en meseta tibial y fémur distal derecho. La biopsia reporta: Condrosarcoma indiferenciado Grado 2. Se inicia sesiones de quimioterapia profiláctica y se planifica la cirugía de salvamento, esto ante la negativa de la paciente para aceptar la cirugía de amputación. La cirugía de reconstrucción se realizó mediante la colocación de una prótesis semiconstreñida tipo Endo Model cementada. En el posoperatorio se realizó controles y curaciones semanales, con buena cicatrización de la herida, y posteriormente rehabilitación por fisioterapia. Actualmente la paciente puede deambular con apoyo y casi de manera independiente.
