Conditional Survival Estimates Improve Over 5 Years for Melanoma Survivors with Node-Positive Disease

Abstract: Conditional survival estimates are more optimistic and realistic for cancer survivors than traditional survival estimates over time. For node-positive melanoma survivors, 5-year conditional DFS and DSS improve significantly over time. These estimates are critical to treatment decisions and non-treatment-related planning for both clinicians and patients.

“…However, with time, the conditional survival estimates demonstrate that the age gradient, or negative impact of age, narrows with time in patients with localized and regional disease. Prior studies have used age 50 as a binomial cut point, obscuring the effects of more advanced age at diagnosis [1819]. One European study did demonstrate an improvement in conditional survival estimates by age, with a narrowing of the age gradient for melanoma patients, but this was not further analyzed by stage [12].…”

“…For example, overall 5-year conditional survival estimates for patients with stage I melanoma do not significantly change in the first 5 years after diagnosis [23][24][25]. In contrast, for patients with stage III and even more notably for patients with stage IV melanoma, estimates of 5-year conditional survival increase in each year for at least the first 5 years following the initial diagnosis of stage III or IV melanoma [23][24][25].…”

“…In contrast, patients with benign nodal nevus aggregates have been shown to present with a 5 year survival similar to that of patients who had no positive findings on sentinel lymph node biopsies [7], [8]. Thus, the differentiation between the nodal nevi and metastatic lymph nodes has a profound effect on the prognosis of the patient, as the recognition of the differences is important to avoid misdiagnosis and provide adequate therapy options [7], [8], [9].…”
